These materials help case managers and stakeholders figure out where to get started. The Case Manager Handbook explains the roles and responsibilities, plus worksheets and evaluations. The Self-Advocate Handbook helps identify goals and create a person centered plan. The Community Support Guide and Orientation Presentation provide an overview of terms and ways to implement personal assistance programs. A one page handout on stakeholder values and vision plus the overview brochure are included.

This unique and innovative handbook is a guide for educators, providers, support people and/or family members to use as they work closely with youth and adults with significant disabilities, mentoring them as they develop and practice self-advocacy skills that will promote the highest level of independence and success in life activities.

This brief piece explains the steps involved in the transition process in Nevada including the services and supports available and the agency that provides referral for those seeking transition from an institutional setting. The goal of the project was to divert people from institutionalization and/or transition people from institutional care into a less restrictive environment.

This document was developed by the Michigan RCSC grantees as part of their Respite for Children program. The booklet explains the concept of respite, who is eligible for the services, the various models of respite, and how to find the right fit for your family. Next the pamphlet describes how to hire a worker and prepare them for the work they need to perform.

This report focuses on individuals with Alzheimer's and other dementias. The project instituted a comprehensive system to support enduring changes in the delivery of community-based assistance for Medicaid eligible persons with dementia. The projects two goals: facilitate hospital-to-home transitions for individuals wishing to stay in the community; and field test a nursing home-to-community transition program for eligible individuals. The workbook used in discharge planner training is included.

The booklet is a publication of the Louisiana Nursing Facility Transition Grant. It is a Self Assessment Transition guide to assist individuals in nursing facilities on how to start planning for a move to the community and obtaining the supports and services they will need. It includes contact information for available state programs, along with contacts for housing options.

The Guardian Mentor Program was set up, in part, to assist legal guardians with issues, concerns and questions that may arise in the first year after someone from a State Center transitions to the community. This brochure explains the guardian-to-guardian mentor program which including support, information and advocacy for legal guardians.

The information included in this booklet is a starting point in educating and informing caregivers of children with disabilities about their rights and creating early expectations and models for self-determination. Also listed are a number of non-profit, cross-disability organizations offering emotional support and a wealth of information on how to get the services you need for your child in Arkansas. This user-friendly guide also includes a dictionary plus a glossary called Alphabet Soup.

This strategic plan outlines the next five years, discusses goals and complements existing Alaska Works Initiative (AWI) activities to 1) address the major barriers that keep people with disabilities from working 2) enhance Alaska’s Medicaid programs and existing employment infrastructures to better meet the needs of working people with disabilities; and 3) imbed what has been learned through the AWI into a comprehensive employment system.

As part of the Georgia QA/QI Real Choice Systems Change grant, an evaluation of the Person-Centered Planning (PCP) program was conducted. Part of this includes the outline for planning and creating PCP groups, and a list of resources for effective use of PCP. Also included is the final survey given to PCP participants and information on sustaining person-centered planning.
