Opportunity Partnership & Empowerment Network: A Case Study of an Effective Employee Retention Project

This report was authored by PHI for the Health Field Collaborative (HFC) in Michigan to bring attention to its approach to improving retention of entry-level workers in health care and direct-care workers in the long-term care industry. The study explains how HFC created the Opportunity Partnership & Empowerment Network (OPEN) to address a critical local need. It outlines components of the program, reviews outcomes for the pilot phase, and identifies factors for sustaining it in the future.

Family Health Care Tips & Fact Sheets – New Mexico

The New Mexico Family to Family grantees, Parents Reaching Out, have put together over a dozen tips sheets. Topics include Advocating for Yourself, Teen Scene: Communication with Health Care Providers, Preparing for Doctors Visits, Waivers, Navigating Medicaid, SSI, Preventing Medical Errors, Transitioning Health Care, HIPAA, and FAQ sabot Mi Via and EPSDT. Also includes is a brochure called Telling Your Story, Tips for Sharing your Experiences. All the fact sheets are also available in Spanish.

Transition/Diversion in West Virginia: A Navigator’s Guide to Community Inclusion

Part of the Transitioning to Inclusive Communities (TIC) Project this “how-to” book is for “navigators,” people who want to assist those who wish to move from a nursing facility to the community. Community Advisors: Roles and Skills

How can a community advisor help find choices and supports? The MA CPASS grantees developed two tools to support self-determination. The first is a checklist of suggested competencies, attributes and skills of necessary to be a Community Advisors. The second resource is a guide defining the role of an advisory, how to use and select the right person, plus checklist of other considerations.

First Steps Handbook: How to get organized to find the help you need

A product of the MASS C-PASS mini-grant project operated by the Multicultural Services Center of the Pioneer Valley (Otro Puente) this bilingual guide was designed for newly arrived Latino individuals and families to the city of Holyoke, MA. Written in response to concerns that many families don’t know how to start looking for supports for a person living with a disability, the guide reviews how to prepare for an interview or home visit, ways to gather information and a list of local agencies.

Partnerships to Independence: Resources for Successful Nursing Home Transitions

Partnerships to Independence (PTI) was created by the Birmingham Independent Living Center in Birmingham, Alabama through a grant from CMS. This transition guide addresses nursing home outreach, support needs and skills, follow-up, and barriers. The purpose of PTI was to develop a model program that could be replicated in other CILs in Alabama and the US. ILC staff identified a number of basic resources and strategies that can be used to promote nursing home transition.
