North Carolina's Division of Mental Health, Developmental Disabilities and Substance Abuse Services has posted a number of great resources to their website related to quality. A sampling of the forms, manuals and recent reports includes: The Quality Management Survey, a Consumer Satisfaction Survey, forms on Critical Incident, Quarterly Reports, and reporting guidelines for local management entities in their annual Quality Improvement Reports.

Family Voices of North Dakota has developed two handbooks. The intent of the booklets is to help families with resource questions and offer a way to record the information they gather. New Beginnings is a guide of referrals and contacts. Care Notebook is an organizing tool for keeping track of important information about a child’s health care. Although this material is specific to North Dakota, other Family to Family grantees may find the guides useful when replicating resources.

Access to affordable, accessible, and integrated housing within a community of choice has been identified as one of the primary barriers faced by persons with disabilities in West Virginia. Lack of housing has become a major barrier to successfully moving individuals from institutions to the community. This manual is intended to be a resource to assist individuals with understanding the complicated maze of systems that must be navigated to successfully own or rent a home or apartment.

This disability preparedness website provides practical information on how people with and without disabilities can prepare for an emergency. It also provides information for family members of, and service providers to, people with disabilities. In addition, this site includes information for emergency planners and first responders to help them to better prepare for serving persons with disabilities.

The Minnesota ADRC utilizes the following two screening tools to assess the long term care needs of individuals and persons with developmental disabilities. Questions include general information, an assessment section, history and professional conclusions.

The Minnesota Association of Centers for Independent Living (MACIL) prepared <i>A Consumer Guide for Planning Your Move from a Nursing Home</i>. This comprehensive guidebook assists with the relocation decisions as consumers transition from nursing homes into the community.

Ohio has shared its Requirements Specification for the CART Test Plan (The Comprehensive Assessment and Review Tool project). The document relates to the development of web-based applications for quality data collection. This Test Plan document supports the following objectives: Perform system testing in a stable test environment; Recommended requirements are that functional business units; validate the application continues to meet stated requirements; usage of a testing matrix checklist

The South Carolina Department of Disabilities and Special Needs has produced a flowchart for the Quality Assurance Review Process. The process is performed by First Health and relates to the overview of the contract.

The WV QA/QI grant designed and developed database as a monitoring tool. This entry includes 5 provider quality monitoring tools: Case Management, Client Home Visit, Homemaker, Personnel Files, Site (Case Management or Homemaker). The database was created by Donna Brewer, Database Administration at the Center for Excellence in Disabilities at WV University for the Quality Assurance & Improvement Project. The Bureau for Medical Services is the lead agency & the grant was funded by WV CMS

The State of West Virginia has shared a number of sample forms for their MR/DD waiver programs. Tools include a variety of evaluations, forms, reports worksheets, summary sheets and applications.
