This study was designed as a small scale, pilot qualitative study of the implementation of Rider 37 in Texas.

The Aging and Disability Resource Center Grant Program, was developed to assist states in their efforts to create a single, coordinated system of information and access for all persons seeking long term support. This toolbox has been developed for ADRC grantees to: 1) List and succinctly explain the AoA-CMS requirements per each topic, and 2) provide materials that will support the design, development and operation of resource centers.

The question “How do we know cultural competence when we see it?” prompted HRSA to sponsor a project to develop indicators of cultural competence in health care delivery organizations. The tool is intended to 1)develop an analytic framework for assessing cultural competence in health care delivery organizations; 2)identify specific indicators that can be used in connection with this framework; and 3) assess utility, feasibility and practical application of the framework and its indicators.

The Texoma Real Choice Program--a demonstration project made possible by a grant awarded to the Texas Health and Human Service Commission by the federal Centers for Medicare and Medicaid Services. The programs goal is to connect persons with disabilities to real service options with the help of system navigators available to assist people through systems and support groups. The below attachments are forms, surveys and other tools needed to implement the program.

The Heart of Texas Regional Access System (HOTRAS) is a network of organizations committed to improving service delivery to people in need. When clients give consent to release information to the network, they agree to participate in the HOTRAS Continuum of Care. The goal is to share and manage information in an effort to coordinate and improve delivery of needed services and to avoid duplication in providing basic intake information. A paper from the Asset-Based Community Development Institute demonstrates how five \"tools\" successfully used for community-building activities may be adapted to help former welfare recipients move toward economic independence and more fulfilling lives. Three of the tools are inventories and two involve support groups.

This guide is a workbook for consumers that are transitioning or wish to transition out of a nursing home. The guide provides information on programs, housing, home modifications, etc. and provides space for the consumer to fill in the steps they need to take in each category. Created for consumers in Alabama, this guide will be useful for consumers in other states as well, although contact information and specifics will not apply.

This document contains statements of conditions that may be pertinent to consider in discussing the viability of a particular service. These statements are not all-inclusive. Each participant is an individual and as such deserves unique consideration of his/her situation when collaborating on a service package.
