Prevention and Risk Assessment Tool

This quality tool product was developed by the state of Arizona, Department of Economic Security. The Prevention and Risk Assessment form identifies risk in a variety of dimensions, including home, work, and the community. The tool identifies risk prevention and mitigation strategies, as well as needed supports and the individuals who can/will provide these supports.

Second Southwestern Regional Forum on Community Inclusion: A Sharing of Ideas on Community Inclusion for People with Disabilities

Discussion at this meeting was lively and productive, and the opportunity to meet people from other parts of the state and other municipalities proved fruitful. Following the suggestion of participants in the October Forum, the agenda included presentations of what the three Model Communities have done and how it is working, and sharing of successes, ideas and resources among other participating towns.

First Eastern CT Regional Forum on Community Inclusion: A Sharing of Ideas on Community Inclusion for People with Disabilities

Discussion at this meeting was lively and productive, and the opportunity to meet people from other parts of the state and other municipalities proved fruitful. The idea of adapting the “lessons learned” in the model communities and hosting regional meetings was introduced; a shared enthusiasm for the idea gave rise to a series of Regional Forums. This forum, held in Groton, CT was the second in the series, held on December 3, 2004.

Needs Assessment for Children with Special Needs - Nevada

The Nevada Children with Special Health Care Needs conducted a 5 month multi-method needs assessment that describes demographics, universal needs and barriers families with children with special health care needs face. Completion of this needs assessment represents an opportunity for community input into the prioritization of needs design & implementation of positive changes to the system of care. The report also includes a summary of Selected States’ Service Delivery Models, tables & figures.

Aging and Disability Resource Center Guidance for Evaluation Plan

This document provides information on elements to include in the ADRC evaluation plan. This resource will provide guidance on what to include, and will serve as an example of how to organize the elements into a matrix, but this format can be tailored by each grantee. Elements to incorporate into the ADRC evaluation plan include: Project Objectives; Tasks; Indicators; Data Sources; Data Collection Methods; Timeline/Interval; Responsible Entity; and Instruments.

Personal Support and Safety Plan Worksheet

This is an example of a personal support and safety plan. This document can be used with clients to determine what they hope to accomplish, design a people map and outline important and positive characteristics about themselves. The plan then focuses on support and what others need to know to provide assistance. Lastly, the plan address what is happening in life (ups and downs) and forms an action plan.

The ARC of Tennessee developed this Personal Assistance Supports and Services Guide to provide people with disabilities the information & tools needed to hire and manage a Personal Assistant. Consumers first complete the self-assessment tool to determining needs. This entry also includes the user's guide and evaluation feedback form.

First Connecticut Regional Forum on Community Inclusion: A Sharing of Ideas on Community Inclusion for People with Disabilities

Expanding upon the \"lessons learned\" from the first Model Communities forum held in Connecticut in May 2004, this Community Inclusion forum held in Ridgefield CT brought together a variety of communities interested in \"enhancing awareness among community members of the barriers faced by people with disabilities in becoming fully included & integrated into communities.\" Participants identified barriers to inclusion and suggested numerous strategies communities can use to address these issues.

Model Communities Forum: A Sharing of Ideas on Community Inclusion for People with Disabilities

On May 25, 2004 representatives from seven Connecticut towns gathered at the University of Connecticut A.J. Pappanikou Center for Excellence in Developmental Disabilities in Farmington to share ideas and experiences related to the inclusion of people with disabilities in the life of their communities. In attendance were representatives from the Model Communities of Bridgeport, Groton and New Haven, as well as four other community representatives from municipalities not selected for funding.
