Wisconsin’s Family Care pilot program constitutes a major re-design of the state’s long-term care (LTC) system. Concerns over system fragmentation, institutional bias, and rising expenditures fueled a strong desire in Wisconsin to improve and redesign the system. This paper summarizes the Wisconsin Resource Center (RC) experience as documented in The Lewin Group’s four reports for the Wisconsin Legislative Audit Bureau’s Evaluation of the Family Care Program.

The three documents provided here focus on elements of ADRC research and evaluation. The documents specifically address data sources, minimum data sets, national evaluation design, potential indicators, and a draft ADRC evaluation framework.

Social marketing is the planning and implementation of programs designed to bring about social change using concepts from commercial marketing. The ADRC Grant Program, a cooperative effort of the AoA & the Centers for Medicare & Medicaid Services (CMS), was developed to assist states in their efforts to create a single, coordinated system of information and access for all persons seeking long term support to minimize confusion, enhance individual choice, and support informed decision-making.

The Medicare/Medicaid Integration Program aims to end the fragmentation of financing, case management, and service delivery that currently exsists between the two programs. A report focusing on integrating information is provided, in addition to a link that provides several technical assistance reports.
