The Center for Housing and New Community Economics (CHANCE) mission is to improve and increase access to integrated, affordable, and accessible housing coordinated with, but separate from, personal assistance and supportive services. CHANCE's purpose is to offer alternatives to approaches that segregate, congregate, and control people with disabilities.

This manual offers local officials and affordable housing advocates tools for overcoming NIMBYism, or the Not In My Back Yard syndrome. The report answers questions such as: \"What is Affordable Housing\", \"Who Lives in Affordable Housing\", \"What Role Does Design Play?\"

This guide is a workbook for consumers that are transitioning or wish to transition out of a nursing home. The guide provides information on programs, housing, home modifications, etc. and provides space for the consumer to fill in the steps they need to take in each category. Created for consumers in Alabama, this guide will be useful for consumers in other states as well, although contact information and specifics will not apply.

The tools in this section, developed by the Michigan Department of Community Health, are intended to help ensure effectiveness and thoroughness. These tools focus on person centered planning and transition planning. Included are timelines and worksheets on housing, transportation, employement, and many other topics.

The Living and Working in the Community 2003 conference, sponsored by the Centers for Medicare and Medicaid Services (CMS), was held March 4-5, 2003. Participants included Medicaid grantees, consumers, advocates, Federal and State representatives, providers, and academics. This resource guide has been developed to assist Maryland residents of a nursing home to have a successful return to living in the community. The guide includes information on many services including: peer counseling, housing, and money management.

This teleconference was intended to provide information about the Supported Housing And Homeless Program, which is part of the Colorado Department of Human Services. The presenter describes the housing authority and how it is organized.

This teleconference focuses on three programs offered by the Department of Housing and Urban Development(HUD. The three programs are-Section 811 Supportive Housing Programs for persons with disabilities; Mainstream housing opportunities for persons with disabilities and Continuum of Care; Homeless Assistance Supportive Housing Programs.

These slides were presented at the 2nd Annual Training Institute for State Mental Health Olmstead Coordinators (9/30/2002 - 10/2/2002). They provide a brief overview of the FY01 and FY02 Systems Change Grants, including charts depicting which states are focusing their efforts in areas such as housing, mental health, nursing facility transition, etc.
