Alabama \"Partnerships to Independence\" Transition Guide

This guide is a workbook for consumers that are transitioning or wish to transition out of a nursing home. The guide provides information on programs, housing, home modifications, etc. and provides space for the consumer to fill in the steps they need to take in each category. Created for consumers in Alabama, this guide will be useful for consumers in other states as well, although contact information and specifics will not apply.
