ADRC-TAE: Streamlining Access Self-Assessment with State Examples

How many hoops does a customer have to jump through to receive services? The goal of this self-assessment is to help ADRCs serve as leaders in systems change in the area of streamlined access. The paper outlines several hoops that individuals often have to navigate to gain access to information about LTC services beginning with the initial call. This report contains examples from current grantees, offers context and can be used as a tool for developing a clear strategy for streamlining access.

Medicaid at a Turning Point: Results From a 50-State Survey on State Budgets and Medicaid Policy Actions for Fiscal Years 2006 and 2007

The annual 50-state survey of Medicaid officials finds an improved economy combined with the implementation of the new Medicare prescription drug benefit has contributed to the lowest rate of Medicaid spending growth in a decade and the fourth consecutive year in which Medicaid spending growth has slowed. This site offers a unique mix of quantitative and qualitative policy information to assist advocates, researchers, and policymakers interested in improving implementation of federal workforce policies. Data is presented in state by state summaries and interactive databases for Workforce Investment Act (WIA), Temporary Assistance for Needy Families (TANF), Pell Grants, Perkins Vocational & Technical Education, Adult Education & Family Literacy, and Wagner-Peyser/Employment Service programs.

Medicaid Options for Model LIFE Account Programs

As part of the 2004 Real Choice grants, New Hampshire and Wisconsin proposed to complete a feasibility study and then develop LIFE Account programs. With no federal legislation creating a savings program, grantees are restricted to modified program that works under existing law. This paper considers six options: Section 1115 Demonstrations, Waivers under Section 1915(c) & 1915(b), Section 1915(i) & 1915(j) State Plan programs, and Section 1937 Benchmark programs and suggests some next steps.

Did You Know, Now You Know training

Provided by Family Voices of Wisconsin, this training is organized into two modules and designed to provide a comprehensive, primary overview of health care and community supports for children and youth. The overall goal of this initiative is to help families access services that will benefit their children. The goal is that one day, they won't hear parents say, \"Why didn't anybody tell me about this before?!\" Presentation slides and evaluations are included.

FY 2001 Nursing Facility Transition Grantees: Final Report

This report is the first in a series of final reports that RTI will prepare to document the outcomes of the Real Choice Systems Change Grants. Real Choice Systems Change Grant Program Fourth Year Report: Progress and Challenges of the FY 2003 and FY 2004 Grantees: (Oct. 1, 2004 – Sept. 30, 2005)

This report describes grant activities in three major LTC systems areas: Access to Long-Term Care Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure. For each of the focus areas the report describes Grantees’ accomplishments, illustrates the challenges and discusses the consumers’ role in the implementation and evaluation of activities. The report uses information provided during the reporting period October 1, 2004 to September 30, 2005.

Money Follows the Person Initiatives of the Systems Change Grantees

This report highlights the work of nine CMS Money Follows the Persons Grantees, with a focus on Texas and Wisconsin. The report describes the initiatives, and discusses policy and design factors states should consider when developing MFP programs, including developing legislation and budget mechanisms for making transfers of funds, assuring availability of services and housing, identifying potential consumers for transition, developing NFT infrastructure, and monitoring and quality assurance.

Innovative Strategies for Strengthening Family to Family Health Information And Education Centers

Family to Family Centers provide information and other supports both to family members and to professionals who serve children and youth with special health care needs. This topic paper highlights innovative strategies developed by individual Family to Family Centers to develop partnerships to expand their reach, to enhance outreach to parents, to serve a culturally diverse clientele, to help youth prepare for transition, and to ensure organizational sustainability.

HOME Participating Jurisdiction\'s Open Activities Reports

A new monthly report from HUD is available on-line and is broken down state-by-state and downloadable in an MS Excel spreadsheet. Participating Jurisdiction (PJs) can use the reports to view open activities in IDIS including activities with 100% of the funds drawn and with a status code of FD, or final draw. PJs can view activities that have been open (OP) for several years with little or no HOME funds drawn. These projects, if not progressing, should be cancelled. The data is updated monthly.
