The Texoma Real Choice Program--a demonstration project made possible by a grant awarded to the Texas Health and Human Service Commission by the federal Centers for Medicare and Medicaid Services. The programs goal is to connect persons with disabilities to real service options with the help of system navigators available to assist people through systems and support groups. The below attachments are forms, surveys and other tools needed to implement the program.

This report presents the findings of a survey of the 50 states and the District of Columbia conducted by the Rutgers Center for State Health Policy/NASHP Community Living Exchange Collaborative to identify states that operate single entry point (SEP) systems and to describe the characteristics of SEPs.

The Heart of Texas Regional Access System (HOTRAS) is a network of organizations committed to improving service delivery to people in need. When clients give consent to release information to the network, they agree to participate in the HOTRAS Continuum of Care. The goal is to share and manage information in an effort to coordinate and improve delivery of needed services and to avoid duplication in providing basic intake information. On August 13, 2002, the Centers for Medicare & Medicaid Services (CMS) released a letter to State Medicaid Directors highlighting promising state practices. It complements the letter Tommy Thompson sent to Governors.

An alert is documented featuring the Kaiser Family Foundation State Health Facts Online \"This new resource contains the latest state-level data on demographics, health, and health policy, including health coverage, access, financing, and state legislation.\" The site link is also provided with information about the states.

This is The National Academy for State Health Policy (NASHP) report entitled State Assisted Living Policy 2002. This comprehensive report examines current regulations and Medicaid reimbursement policies for assisted living facilities in each of the 50 states. The report includes detailed comparisons of state policy and a summary of each state's assisted living regulations.

This document is an Information Bulletin (# 54) on subsidized housing. The report includes a weblink and information on a \"new\" source of information available that can assist disability advocates find affordable and accessible units.

On July 16 and 17, 2003, representatives from a diverse assortment of states came together in Denver, Colorado, to share information, learn from one another and explore ways to help each other achieve an important, mutual goal: to build better and stronger communitiesin which people with disabilities are readily and routinely part of the fabric of everyday life. This is a report on that discussion.

These slides were presented at the 2nd Annual Training Institute for State Mental Health Olmstead Coordinators (9/30/2002 - 10/2/2002). They provide a brief overview of the FY01 and FY02 Systems Change Grants, including charts depicting which states are focusing their efforts in areas such as housing, mental health, nursing facility transition, etc.
