Coordinating Existing Human Service Transportation Resources: Maximizing Resources And Economic Benefits

Transportation is an essential community element that is all too often overlooked. In New Hampshire, the failure to prioritize transportation issues at the state & community level is difficult to understand for two primary reasons. First, unmet need. Secondly, inefficiently spending on transportation with the result being duplicative efforts, lack of coordination among service providers, and a weak infrastructure. National studies show that fixing the second may, in fact, fix the first.

Work Book: Improving Quality in Home and Community-Based Services and Supports

The HCBS Work Book is a technical assistance guide to assist state agencies with their HCBS quality improvement efforts. It provides step-by-step guidance on how to conduct quality improvement projects within the HCBS waiver program, and illustrates the principles of quality improvement in an easy to use format. The Work Book was developed for CMS by the Muskie School of Public Service at the University of Southern Maine.

New Hamphire Governor\'s Commission on Disability Website

The Commission's goal is to remove the barriers, architectural or attitudinal, which bar persons with disabilities from participating in the mainstream of society. In order to lead full and productive lives, persons with disabilities need employment, housing, education, recreation and other opportunities now available to persons without disabilities.

Technical Assistance Provider Contact Information

Beginning in 2001 and continuing in 2002 and 2003, technical assistance for the Real Choice Systems Change grantees has been provided by the Exchange Collaborative team (combined efforts of ILRU and Rutgers University/CSHP). These documents provide contact information for the TA providers and two charts indicating which Exchange Team is responsible for providing TA to the grantees. In 2002 & 2002, TA was divided according to states. In 2003, TA was distributed according to theme.

Alpha One Organization for People with Disabilities

Alpha One, a Center for Independent Living, has enabled thousands of people with disabilities to live more fully and more independently in all aspects of their lives. The Future Supply of Long Term Care Workers in relation to The Aging Baby Boom Generation

This report is a product of collaboration between HHS and DOL in response to the requests from the U.S. Congress. Staff at these Departments worked collaboratively to share information and data pertaining to direct care workers in long-term care settings and to develop a joint set of recommendations for the future. This report includes information on informal caregivers, direct care workers, and paraprofessional direct services workers.
