This report describes the FY ‘01 & FY ‘02 Grantees’ accomplishments & progress, using information from 10/1/02 to 9/30/03 (Year Two of grant period for FY ‘01 Grantees & Year One of grant period for FY ‘02 Grantees). The report describes grant activities in six major areas: Consumer Direction & Control, Access to LTC Services & Supports, State Budgeting & Reimbursement Rates & Methodologies, Service Creation/Modification, LTC Service & Support Workforce, Quality Management Mechanism.

Real Needs-Real Costs With an Eye Toward the Future: A Process for Developing Service Plans and Individual Budgets was presented on October 12, 2004 at a teleconference for CMS Independence Plus Grantees. The presentation draws on 6 years of prior experience in developing Consumer Directed Programs for adults at the Area Agency of Greater Nashua.

On October 12, 2004 the Community Living Exchange Collaborative at Rutgers Center for State Health Policy hosted a Self-Direction audio conference for Independence Plus and CPASS grantees to discuss Person-Centered Planning and Individual Budgeting. Presentations inlcuded CMS, Minnesota, Wyoming, and New Hampshire.

On October 5, 2004, HHS Secretary Tommy Thompson announced $31 million in grants to 31 states to help people with disabilities or long-term illnesses live in their homes and participate fully in community life. The grants were awarded under the New freedom Initiative and are known as Real Choice Systems Change Grants. A total of 52 grants were awarded in nine categories. This job description is for the New Hampshire Aging and Disability Resource Center seeking Advisory Group members. The role of the Aging and Disability Resource Center Advisory Group (ADRC) is to advise the State on the design and operations of the Resource Center.

Included here are several documents pertaining to New Hampshire's ADRC Advisory Board. These documents include: A participant evaluation form, a member invitation letter, kickoff meeting materials, and a cover letter to consumers.

This transcript is from a call on \"Emergency Back-Up and Independence Plus\". Participants on the call discussed the requirement for states to have an Emergency Back-Up System at both the individual level and the systems level. Grantees heard examples from states in action, which included New Hampshire, Louisiana, Florida and South Carolina. These states discussed what they are doing around emergency back-up and some of the lessons that they have learned.

This report describes the Commonwealth of Pennsylvania’s Office of Mental Retardation's efforts to implement a system-wide reform of its supports for people with mental retardation. The objective of this Transformation Project is to improve the quality of life of people who use publicly funded supports by providing them with more choices and greater control over these supports.
