Lessons from the Implementation of Cash and Counseling in Arkansas, Florida, and New Jersey

Cash & Counseling provides a flexible monthly allowance (based on the consumer’s care plan or claims history) that consumers can use to hire their choice of workers, including family members, and to purchase other goods & services. A demonstration was implemented in three states-Arkansas, Florida, & New Jersey. Based on their experiences, this paper draws lessons on designing & implementing a Cash & Counseling program, to provide information useful to states thinking of adopting such a program.

HHA Partnering Collaborative Evaluation: Practice/Research Brief

The VNSNY HHA Partnering Collaborative was designed to better integrate professional and paraprofessional services and employ established principles of quality improvement to achieve two main goals: improving the quality of work life of home care paraprofessionals, and increasing nurses' and aides' support for patients' improvement in key ADLs. This brief describes the key components of the model, implementation challenges and successes, and the results and implications from the evaluation.

Sustaining NFT: Covering Care Management.

During this teleconference on 11/10/2003, Mary Jean Duckett from CMS presented answers to the grantee question: \"How do we continue working with people in nursing homes after our grant ends.\" Three options were discussed: Transition can be covered as a service under the Home and Community-based waivers; as a targeted case management service under the state plan or reimbursed as an administrative activity.

Promising Practices in HCBS: Ohio- Increasing Timely Access to Services

A home and community-based services program in Ohio is using electronic communication between case managers and providers to streamline identification of service providers for program participants. Program staff have found electronic communication greatly reduces the time required to find available providers when compared with telephone or fax communication. This report explores the interventions, implications, and impact of using electronic communication.

Promising Practices in HCBS: Rhode Island- Case Management that Follows Offenders Out the Prison Gates

Promising Practices in HCBS: Illinois- Simplified Access for HCBS Waiver and Older Americans Act Services

This report describes how the State of Illinois simplifies access to home and community-based services for people with disabilities age 60 and older. Local Case Coordination Units coordinate eligibility determination and case management for Medicaid waiver services, state-funded home and community-based services, and Older Americans Act services. This report explains the implementation and impact of this program.

Workforce Strategies: Introducing Peer Mentoring in Long-Term Care Settings

This issue introduces long-term care providers to peer mentoring, an innovative strategy that has been shown to significantly improve retention among direct-care workers. Implementing peer mentor programs at home care agencies and in nursing facilities, we identify the benefits of mentoring programs, define the peer mentor’s role, discuss critical mentoring skills, & outline the key design elements that long-term care organizations need to consider when developing their own peer mentor programs.
