IDA: Creating a Virtual Information Service to Meet Maine’s Need for Integrated Data

This document describes IDA, a proposed generic, virtual information service, supporting both operational (e.g., client service) and analytical needs for integrated data. In IDA, source data would stay with the agency that owns it and be made available to an end-user only when permitted by the owning agency. IDA responds to many of the persistent barriers to data integration that confront the State.

North Carolina\'s Information and Referral Vendor RFP

North Carolina issued a Request for Proposal notice for an information and referral vendor to serve the ADRC program in February 2005. The purpose of this RFP is to solicit proposals for the implementation of an integrated cost-effective commercial-off-the-shelf (COTS) solution that will provide a scalable and flexible Information & Referral (I&R) data management system that incorporates both a central web-accessible I&R data repository and individual I&R services, with minimal customization.

Pennsylvania Community Choice ADRC Survey and Assessment Instrument

The Pennsylvania ADRC program has developed a number of measurement instruments including three surveys and an assessment instrument. This program is a joint effort between Pennsylvania’s Office of Health Care Reform as well as the Departments of Aging and Public Welfare. The products include a survey for eligible, ineligible and enrolled consumers and a community choice assessment instrument.

Memorandum Of Understanding (MOU) Regarding Family Care Eligibility Determination And Enrollment- Wisconsin

The MOU is developed among the ADRC in Richland County, the Economic Support Services Unit (ESS), the enrollment consultant for WI Family Care program, and the Care Management Organization (CMO). The MOU is intended to help clarify the specific roles and responsibilities, ensure that the participating units understand the key elements of coordination and communication, and to facilitate their working in partnership to achieve quality outcomes for program consumers and the general public.

Maine’s Work Group for Community-Based Living includes a broad array of consumer and state representatives from five departments (Transportation, Labor, Education, Human Services, and Behavioral and Developmental Services). The Unequal Availability of Affordable Assisted Living Units in Florida’s Counties

The supply of affordable assisted living facilities is now insufficient to meet the demands of low-income, frail older persons. This gap between demand and supply is much more apparent in some locations than others. This article uses various measures of locational inequality to assess the extent to which counties in Florida have less than their fair share of ALFs, particularly in comparison to the locations of Medicaid nursing home beds.

Housing Search Guide: Relocation from Nursing Facilities to the Community 2003

The “Housing Search Guide” was developed to assist professionals who have a role in “relocation” of people with disabilities from nursing facilities to community living arrangements with appropriate services and supports. The Guide was developed by staff of the Texas Independent Living Partnership, a program of ARCIL, Inc. funded by the federal Centers for Medicare & Medicaid Services under the “Real Choice Systems Change” program.

Rebalancing the Texas Long Term Care System: A Blueprint for Systems Change

The purpose of this project is to promote transition from an institution-based to a community-based system, in keeping with the goals of the federal New Freedom Initiative. This document summarized the policy recommendations of the Texas Independent Living Partnership over the three-year grant period, providing a resource to Texas state agencies, Centers for Independent Living, and other interested parties for ongoing systems advocacy and program development.

Challenging Cases Protocol

Washington developed this protocol for use when the recommended plan of care can not assure the health and welfare of recipients due to participant issues, environment, or resources. Addresses shared responsibility for participant safety. Addresses service denial and service termination. The purpose is to promote statewide consistency in dealing with difficult to serve clients before denying or terminating services.
