The West Virginia Inclusion Campaign Materials

The West Virginia Inclusion Campaign was created as an initiative to counteract the effects of social bias and discrimination against people with disabilities or people living with long-term illnesses. Part of the project has been to develop outreach materials. Please review their program brochure, Disability Awareness Guide and Reporter’s Guide.

Communities for Everyone: Universal Design for Community and Systems Change

Communities for Everyone is a two-day institute held June 23rd and 24th that will explore innovative approaches to building inclusive communities and creating sustainable systems change to support all residents to be full participants in community life. Workshops and keynote presentations will provide information that reflects current practices as well as future designs to create welcoming, accessible, and inclusive communities that support all residents.

Children with Special Health Care Needs Media Campaign

The Children with Special Health Care Needs programs of Nevada assists children with special needs and their families through a variety of health and support services. The Real Choice Systems Change Project has partnered with the Nevada Broadcasters Association to air the various media information on Nevada's television and radio stations. They also host a website with links to support, advocacy, assistance and education.

Forsyth County Aging and Disability Resource Initiative Update Site

The Forsyth County North Carolina ADRC has shared their advisory council insite. This website is an on line resource for council members. The website was designed to keep the local advisory council updated on meetings and obtain copies of important documentation. The site offers the RFP, the grant application, acronyms, fact sheets, work plans, work group descriptions, meeting minutes and more.

Personal Care Assistance (PCA) Program: Consumer Guide Book

This guidebook is written to provide information about Minnesota’s Personal Care Assistance (PCA) program and the many options consumers have. The state strives for a consumer directed service model. Sections in this guidebook define services, help consumers develop skills and provide resources to help take more control of the important services that allow independent living. This guidebook is current as of September 2004.

Passport to Independence - Delaware’s Nursing Home Transition Program

Passport to Independence is a federally-funded pilot project led by Delaware Health and Social Services’ Division of Services for Aging and Adults with Physical Disabilities (DSAAPD). The overall goal of the program is to identify, inform and assist nursing home residents, especially those who are Medicaid-eligible, who want to move to a new home by offering individualized case management to accomplish the goal. Hospice Survey of West Virginia

A survey in West Virginia was conducted in February-March of 2005 regarding: 1. Hospice programs that serve adults with developmental disabilities (DD) 2. training and educational materials for hospice staff on caring for people with DD 3. information about hospice end-of-life care developed for service provider staff and families. The national information gathering activity was initiated by the Association of University Centers on Developmental Disabilities. This report reviews the results.
