Ohio\'s Quality Framework

What areas of quality are important to individuals with disabilities, families and stakeholders in Ohio? The framework illustrates the areas of quality to be addressed by a functioning quality information management system. Organized by Domains, each section is accompanied by an Outcome. Next, the Core Indicators express how the system will know if the Outcome is present. Performance Measures establish the criteria for measurement and calculation. Implementation of the Quality Management Initiative- Communication Plan: Request for Proposals

The Ohio Department of Mental Retardation and Developmental Disabilities (ODMRDD) Division has issued an RFP for a contractor to implement recommendations received through another consultant for their Marketing and Communication plan and to implement a recently completed Communication Planning Tool. The plan is specific to educating all constituents on Quality Management concepts and how information resources will assist in making decisions affecting quality of life. RFP closes 10/20/06.

ADRC National Vision and State Models Presentation

The National Association of Area Agencies on Aging (n4a) Conference was held in August 2006 in Chicago. A one day session focused on Aging and Disability Resource Centers. The day was broken into three sections: an AoA/CMS Overview, State Profiles from Ohio, Indiana, Iowa and Illinois and an audience consultation session. Review the overview slides and the state summaries which explore Single Point of Entry and navigating the long-term care systems maze.

Seeing Seniors: Shaping The Future of Aging in NH –Video Clip

ServiceLink, the New Hampshire ADRC grantee, along with New Hampshire Public Television and Easter Seals NH/Seniors Count have developed a year-long community outreach project. The partnership resulted in six feature stories on a nightly newsmagazine and community forums in four locations across the state bringing seniors and caregivers together with local leaders to begin planning the kind of communities they want as they age. Community Options- Development and Implementation of a Continuing Education Seminar

This presentation was developed by the Maryland Department of Health and Mental Hygiene as part of their Real Choice Systems Change Grant. The grant created and executed a Discharge Planning Resource Seminar to educate discharge planning professionals about basic Medicaid services as well as other State-run and privately-funded home and community-based services. The attached presentation reviews the progress, outcome and evaluation of the outreach efforts.

Utah Care Notebook

This Care Notebook is an organizing tool for families who have children with special health care needs. Utah created the book to help families keep track of important health care information, list providers and community organizations contact information, prepare for appointments and file and share health history. Utah Family Voices shared this guide so that other resource centers may use this as a template when developing a similar tool.

Did You Know, Now You Know training

Provided by Family Voices of Wisconsin, this training is organized into two modules and designed to provide a comprehensive, primary overview of health care and community supports for children and youth. The overall goal of this initiative is to help families access services that will benefit their children. The goal is that one day, they won't hear parents say, \"Why didn't anybody tell me about this before?!\" Presentation slides and evaluations are included.
