MI-CEO Presentations on Employment of People with Disabilities in Massachusetts

A series of presentations, funded by the MI-CEO grant, discuss a variety of issues related to the employment of people with disabilities in Massachusetts. Topics covered include: rates of unemployment, barriers to employment, ways to overcome barriers, the relationship of programs and services (e.g., MassHealth and Medicaid Buy-In) to employment efforts, recommendations from agency leaders, and implications for MI-CEO.

Employment, Disability and Medicaid-Buy-In Program Reports: Massachusetts

Developed by the MI–CEO Grant, a report on the employment of people with disabilities in Massachusetts intends to provide people with disabilities, advocates, policymakers, researchers and other interested parties an overview of the status and trends in employment of people with disabilities. Another report uses new longitudinal, person-level data to support the analysis of the Medicaid Buy-In (MBI) programs in Massachusetts.

South Carolina Training Presentations: Long-Term Care Planning for Seniors and Planning for Parents of Children with Disabilities

The SC ADRC project developed these two PowerPoint Presentations used to 1) educate and train adults in South Carolina about the importance of and strategies involved in planning for long-term care needs in the future and 2) train parents or relatives of children with disabilities about future planning for themselves and their children.

Funding and Service Recommendations for Transitioning Older Adults: An Examination of Illinois’ Money Management Participants

This report from the Illinois RCSC Rebalancing Grant, quantifies the actual expenses faced by older residents living in the community and applies it to those living in a nursing home seeking reintegration to community living. Furthermore, it hopes to identify a floor of income required to sustain community residency. The analysis should be helpful in formulating system changes for programs including eligibility standards and policies.

Transition/Diversion in West Virginia: A Navigator’s Guide to Community Inclusion

Part of the Transitioning to Inclusive Communities (TIC) Project this “how-to” book is for “navigators,” people who want to assist those who wish to move from a nursing facility to the community. The guide gives basic information, plus supportive documents, tools, forms and other resource materials. Sections include: Navigator Roles and Responsibilities, Individualizing Transition/Diversion, the Transition Process, Building a Team, Tools for Success, Responsibilities, Rights and Resources.

Supporting Transitions for EPSDT Eligible Children in Foster Care with Emotionally Based Problems in the District of Columbia

The focus of this grant is youth in the child welfare system ages 15–21 years with significant emotionally based problems. The goal to ensure that the “aging out” foster youth, transition into adult support and services smoothly and appropriately - focusing on the youth’s choice of living environment, provider of service, and quality care. The project was funded Sept. 2004-Sept. 2007. Review the projects goals, activities, status, core elements, gains and challenges, and steps to moving forward.

Community Advisors: Roles and Skills

How can a community advisor help find choices and supports? The MA CPASS grantees developed two tools to support self-determination. The first is a checklist of suggested competencies, attributes and skills of necessary to be a Community Advisors. The second resource is a guide defining the role of an advisory, how to use and select the right person, plus checklist of other considerations.
