Real Choice Systems Change

This is the first of three national teleconferences that addresses strategies for improving home and community based services for individuals with mental illness. This call looked at promising practices using the Medicaid Rehab option.

On July 16 and 17, 2003, representatives from a diverse assortment of states came together in Denver, Colorado, to share information, learn from one another and explore ways to help each other achieve an important, mutual goal: to build better and stronger communitiesin which people with disabilities are readily and routinely part of the fabric of everyday life. This is a report on that discussion.

The Formative Evaluation of the Real Choice Systems Change Grants evaluates how the Systems Change grantees are progressing towards meeting their goals. The Formative Evaluation identifies problems faced by the grantees and ways that they deal with the problems. The purpose of this report is to describe the progress fiscal year (FY) 2001 Grantees have made on their activities during the first year of the three-year Grant period (October 1, 2001 through September 30, 2002.)
