This document sets forth the State of Nebraska’s plan on aging in accordance with Older Americans Act requirements. The plan describes the mission and strategies of Nebraska’s State Unit on Aging (SUA) to coordinate services for older adults. The Plan contains information on aging demographics, the mission for the duration of the plan, and sets out goals to empower older adults to be more independent, promote wellness, and expand home and community-based services.

The Plan contains information on how Minnesota aims to achieve its mission of increasing the population of individuals supported at home and in their communities in order to assure people are living as independently as possible.

This document sets forth the State of Michigan’s plan on aging in accordance with Older Americans Act requirements for FY 2011-2013. The plan describes the mission and strategies of Michigan’s Office of Service to the Aging (OSA) to coordinate services for older adults. Amongst many of the goals in the Plan are enhancing individual control through person-centered services, establishing partnerships between stakeholders, and improving overall effectiveness and quality of the aging network.

This document describes the State of Massachusetts’ plan on aging in accordance with Older Americans Act requirements. The plan describes the mission, objectives, and strategies of Massachusetts’ Executive Office of Elder Affairs to coordinate services for older adults. Amongst many of the goals in the Plan for FY10-13 are to address potential homelessness in elder populations, promote consumer directed care, and increase advocacy through the Long-Term Care Ombudsman Program.

This document describes the State of Maryland’s plan on aging in accordance with Older Americans Act requirements. The plan describes the mission, objectives, and strategies of the Maryland’s Department of Aging to coordinate services for older adults. Maryland’s department has a mission to enable older citizens to age with independence and dignity. This Plan describes Maryland’s aging demographics as well as different programs and approaches for its aging population.

This document describes the State of Maine’s plan on aging in accordance with Older Americans Act requirements. The plan describes the mission, objectives, and strategies of the Maine Office of Elder Services to coordinate services for older adults. The Maine plan focuses on expansion of home and community-based services, use of Aging and Disability Resource Centers (ADRCs), and an increase in awareness of elder abuse, neglect, and exploitation.

This document describes the State of Louisiana’s plan on aging in accordance with Older Americans Act requirements. The plan describes the mission, objectives, and strategies of the Louisiana Office of Elderly Affairs to coordinate services for older adults. The Louisiana plan focuses on expansion of home and community-based services, use of Aging and Disability Resource Centers (ADRCs), and consumer choice and autonomy.
