Money Follows the Person

This Executive Summary and accompanying Abbreviated State Reports encapsulates the comprehensive case studies that Congress directed CMS to perform as a first year task in a 3-year study of state management practices in balancing long-term care systems. The studies were undertaken in 8 states (AR, FL, MN, NM, PA, TX, VT, WA) and examine “rebalancing” from a qualitative and quantitative perspective.

The CMS Systems Change conference, Transforming Systems: Keys to Success was held April 10-12 in Baltimore. Presentations from the 2006 sessions are available in both PDF and text formats. Topics range from ideas on system transformation, stakeholder involvement, coalition building, strategic planning, sustaining systems change, and the Deficit Reduction Act. This summary of the Home and Community Based Services (HCBS) State Plan Amendment (SPA) option is based on a review of the Deficit Reduction Act (DRA) and discussion from the April 3, 2006 conference call with CMS staff and Real Choice grantees. The content reflects the Rutgers/NASHP TA collaborations understanding of the legislation and questions raised by grantees. The intent is to highlight what is known about the DRA and some questions about how it will affect States that elect this option.

This paper offers explanation and analysis of the changes to the long-term services policies including: Asset Transfers, Long-term care Partnership Programs, Family Opportunity Act, Money Follows the Person Demonstration, State Option to Provide HCBS Services, Cash and Counseling Option.

The Compendium contains basic information about each of the Real Choice Systems Change Grantees. The Compendium will help you learn more about how these grants will be used to allow more people of all ages with a disability or long term illness to live and participate in their communities. Real Choice Systems Change Grantees will also find the Compendium useful to identify other Grantees with similar goals and activities. The fifth edition of the Compendium replaces all previous editions.

The New Mexico legislature has passed the \"Money Follows the Person Act\" (MFP) allowing people with disabilities of all ages to choose community living. The passage of MFP dove-tails with federal legislation signed into law by President Bush in the Omnibus Budget bill. The New Mexico legislation will allow a person in a nursing facility to choose community living, and have the money that has supported the person in the facility to follow the person to the community.

The Nursing Home Transition Summit, convened by Rutgers Center for State Health Policy, was held September 28 through September 30, 2005. It brought together 39 participants from ten states, in addition to representatives from CMS, to discuss how to sustain nursing home transition programs. The New Jersey Community Choice Counseling program served as the summit centerpiece, with presentations by program staff and visits to several sites in the Central New Jersey area.

This toolbox was developed for nursing facility transition (NFT) and Money Follows the Person (MFP) grantees to provide materials to support the design, development and operation of nursing facility programs. This toolbox includes over 30 resources to assist states in these efforts. Resources include existing materials or tools, CMS guidance and report developed by the Rutgers/NASHP team and others. An electronic link to the tool or document is included when available.

This memorandum presents state-by-state data on Medicaid long term care expenditures in Federal Fiscal Year 2004 (October 2003 through September 2004). The accompanying tables present data on Medicaid Expenditures for Long-Term Care services from 1992 to 2004. A new table this year shows the effect of supplemental nursing facility payments in some states on expenditure data. A separate file ranks states based on the proportion of LTC spending for community services.
