Pennsylvania Employment Resource Guide for Persons with Disabilities

The Pennsylvania MIG project developed the Work Incentive Planning Assistance (WIPA) Program in order to promote employment among those receiving SSI/SSDI benefits. WIPA produced the following newsletter entitled “Employment Resource Guide for Persons with Disabilities”, which sought to succinctly list services available to individuals with disabilities who wish to work. Some of these resources include Family Savings Accounts and information about the Volunteer Income Tax Assistance Program.

Information & Assistance Handbook: Eldersource

This handbook documents the components of a successful information and assistance system. It is intended for use as a guide for service providers to develop or enhance their own I & A processes or to replicate the system utilized at Eldersource. Real Choice Systems Change Grant Program - FY 2001 Community-Integrated Personal Assistance Services and Supports Grantees and Real Choice Grantees: Final Report

This report provides an overview of 35 states’ initiatives to improve their LTC systems and includes lessons learned and recommendations that can guide states undertaking similar initiatives. Despite accomplishments and improvements, Grantees described continuing barriers including insufficient funding for home and community services; workforce shortages; lack of affordable and accessible housing and transportation; and outdated or inflexible administrative, statutory, and regulatory provisions.

UCare: A Website for Utah Caregivers

The UCare website is a feature of the Utah Department of Human services and exists primarily to help caregivers and provide link to other resources. Resources are for the elderly, people with mental illness, for people with disabilities and their caregivers. Information is available on-line and through a statewide toll free number. Other features to note include provider report cards, guides, a glossary and the long-term care planning tool.

Homeownership for People with Disabilities Forms

The Illinois Homeownership Coalition for People with Disability (HCPD) has a comprehensive set of documents pertaining to its advocacy and assistance for homeownership. The organization’s goal is to assist people with disability in purchasing their homes, allowing them to maintain independence and living in their communities. Review the application, brochure and checklist.

The Aging and Disability Resource Center (ADRC) Demonstration Grant Initiative -Interim Outcomes Report

Forty-three states and territories have received three-year competitive grants since the ADRC program was launched. This report details findings at the overall ADRC level and the pilot level on the outcomes, accomplishments, and contributions of the ADRC program over the grant period, including more immediate results related to key consumer and program outcomes. It also documents lessons learned and program and policy implications at the pilot, state, and national level.

Checklist for Enhancing the Participation and Input of People with Disabilities

In many ways, consumers are the best experts on their disabling conditions, including their own needs for support and the effectiveness and quality of programs. As a result, obtaining consumer participation and input at all stages of program design, implementation, evaluation and quality assurance is important. Effectively and efficiently involving consumers and obtaining their input, however, requires creative and innovative methods. This checklist will help to enhance full participation.

Intake and Referral Information: Bangor ADRC

The Bangor Eastern Agency on Aging ADRC project has shared a number of resources involving the Intake and Referral Process. The first is their policies which provide staff guidelines for consistency. The second resource is their intake questions to help record and collect information. Lastly, they have shared their feedback referral process and how to make this simple and accessible by the consumers.

Activities and Accomplishments of the Family to Family Health Care Information and Education Center Grantees

This paper describes the activities of the 19 Grantees funded in fiscal year 03 and 04 from CMS and six Grantees funded in fiscal year 02 by the Maternal and Child Health Bureau (MCHB). F2F Health Information Centers assist families of children with special health care needs. This paper provides information states and stakeholders will find useful when planning or implementing similar initiatives. Information & Referral Database Inclusion/Exclusion Criteria

The Florida Department of Elder Affairs has set forth inclusion and exclusion guidelines for the integrated statewide information and referral database. This database will serve as a single point of access for information, counseling, referrals, assessment and eligibility functions, which will maximize consumer choice. The database includes entities that serve both Florida’s elders and their families, and adults suffering from severe and persistent mental illness.
