Aging and Disability Resource Center

This matrix represents the AoA Aging and Disability Resource Center Fiscal Year 2003 Grantees. The chart summarizes current or planned components for the ADRC Program for each grantee including Project/RC Name, State, Target Population, Eligibility Determinates and Contact Information.

This document provides an overview of essential elements of a non-profit business plan for the ADRC. Sections II and III are written to serve as a template that can be easily adapted by each ADRC. This should only require filling in information in certain sections and modifying text to better fit your effort. Sections IV through VIII contain discussions and tools to complete each of these sections. They will require more effort and original writing to complete.

The ADRC program calls for aging networks to broaden its target population & closely coordinate with the disability network to establish seamless systems for consumers. This brief provides background information on characteristics of individuals with physical disabilities, describes the major information & service needs & the existing supports & services system. The brief also addresses implications for grantees in serving this population & provides additional resources to facilitate operations.

This material outlines the services available at the Milwaukee Department on Aging. The Handout and Resource Center Page provides an overview of the services offered. Also, there are sample marketing materials and flyers for the flu program, the energy assistance and fitness center.

Offered at the 29th Annual n4a Conference in Atlanta. Presentation includes national overview and vision, role of TA, and questions posed to three AAAs developing ADRCs during a July 11, 2004 n4a intensive about ADRCs.

This comparsion matrix contains the information from 12 ADRC programs and the questions that appear on their customer satisfaction instruments. The surveys are used by ADRC-like entities used in developing recommended customer satisfaction questions.
