Promising Practices: Managing the Care of People with Disabilities

The goal of this paper is to identify programs around the country associated with serving and coordinating the care of individuals with disabilities. CHPR conducted a literature search, a series of interviews with national experts, & a review of 6 promising practices. Included in the review: planning; implementation; eligibility; funding and authority; contracting; delivery system and services offered; care management and care coordination processes; evaluation and outcomes; and replication.

Making a Difference is a quarterly magazine of the Governor’s Council on Developmental Disabilities. Information Bulletins for ICF-MR Restructuring for Guardians of Individuals with Developmental Disabilities Currently Living in an Intermediate Care Facility or Nursing Home

These six bulletins provide guardians with answers to frequently asked questions about changes to law that created the ICF Restructuring initiative requiring that community plans be developed for residents of ICF-MRs and that these placements be reviewed by courts annually to determine the most integrated, least restrictive setting for people with developmental disabilities.

Examination Of Texas Rider 37: A Medicaid “Money Follows The Person” Long-Term Care Initiative

This report provides a brief overview of a research study and its methods for both the field research and the quantitative analyses of the Texas Rider 37 program. The authors then present findings and observations from the qualitative study and quantitative analyses relevant to specific research questions. The report concludes with a summary of the findings.

You decide. Your help. Consumer Directed Video

Interested in how to use media to market your program? Minnesota has developed short video clips to make it easier for consumers to learn about “You Decide. Your Help.” The following video segments show viewers how to use the consumer directed service option to provide the help you need to continue to live at home. These clips give viewers the basic information on what the option is, who is eligible and who to contact for more information.

The Transition Planning Process

This data brief provides a national view of the transition planning process undertaken during high school with and for youth with disabilities as they prepare for life after school. The information reported comes from a mail survey of school personnel conducted as part of the National Longitudinal Transition Study-2 (NLTS2). Findings from NLTS2 generalize to youth with disabilities nationally who were 13-16 years old, to each of 12 federal disability categories, and to each age group.

Adult Service Agencies’ Summaries - Utah
