The Department of Health in the District of Columbia used this template to find a contractor to operate a screening and counseling program regarding the long-term care (LTC) and chronic care needs for persons with a physical disability and elderly residents of the District. The operation of this program included developing and managing a centralized Resource Center. The District contemplated award of a cost reimbursable contract.

This report chronicles New Jersey's experience in order to assist policymakers in other states who need to meet their older adults' preferences for long-term care and, at the same time, to develop sustainable services and systems. This brochure is a marketing material for the program Passages in Arkansas. This program offers help to Medicaid residents in nursing homes who wish to return to the community.

The development of this Best Practices Manual is a pilot project to demonstrate a step-by-step process of re-integrating long term nursing home residents from residential living back into the community. This innovative Project is the result of an initiative made possible under the Passages Program sponsored by the Arkansas Division of Aging and Adult Services, in 2000-2003.

This document highlights North Dakota’s plan to pay family caregivers for services. North Dakota, as part of its efforts to increase alternatives to nursing homes, uses state funds to provide monthly payments to spouses and other family members to care for low-income people with disabilities, including older people living at home. The Texoma Real Choice Program--a demonstration project made possible by a grant awarded to the Texas Health and Human Service Commission by the federal Centers for Medicare and Medicaid Services.
