Increasing Options for Self-Directed Services Initiatives of the FY 2003 Independence Plus Grantees

This report describes the activities of 12 Grantees that received Independence Plus (IP) grants in FY 2003 and are using them to increase self-directed services options for persons of all ages with disabilities or chronic illnesses. Grantees encountered a range of issues while implementing. This report provides information for states and stakeholders planning, implementing, or expanding self-direction programs, whether through solely state-funded programs or the Medicaid program.

Empowerment of Native Americans with Disabilities Conference

The Montana Medicaid Infrastructure Grant held the Empowerment of Native Americans with Disabilities Conference on November 2, 2006. Some of the topics addressed included: barriers to employment, social security benefits planning and vocational rehabilitation. Pathways to Employment: Strategic Plan, Work Plan and Brochure

Supported by a CES - Medicaid Infrastructure Grant, Minnesota developed a strategic plan to increase competitive employment of people with disabilities, called Pathways to Employment. The strategic plan describes how Pathways to Employment, through its partners and stakeholders, will work together to build a more comprehensive, coordinated system of employment services and supports for Minnesotans with disabilities. Additionally, the brochure and work plans for 2006 and 2007 are included.

Minnesota Comprehensive Employment Opportunities - Medicaid Infrastructure Grant: Strategic Plan & Funding Application

Developed by the Comprehensive Employment Opportunities – Medicaid Infrastructure grant, a strategic plan project was initiated to increase the competitive employment of people with disabilities in Minnesota. The report, based on data obtained through individual interviews and focus groups in 2005, is included. Additionally, the Continuation of Funding Application for 2006 is available.

Partnership Educational Presentation and Handouts: Oklahoma

As part of the Oklahoma Real Choice Systems Change Grant, Oklahoma developed a number of training tools for their partnership members. The group consisted of Consumer Consultants and Grant Partners. The presentations and handouts describe roles, working as a team, communication, decision making, policy history, state specific divisions and services and the ADvantage program.

RFP for a qualified contractor(s) to complete the National State Profile Tool for Minnesota’s long-term system

The Minnesota Department of Human Services, through its Continuing Care Administration, is seeking Proposals from qualified Responders to work with DHS staff and key stakeholders to complete a State Long-Term Care Profile. A State Profile tool was developed by Thomson Medstat under contract with CMS. The Profile must describe the state’s long-term care delivery system for 7 population groups that account for the majority of people who need hcbs in MN. Proposals are due January 28, 2008.

Georgia Aging and Disability Resource Connection Expansion Evaluation

This report describes the process evaluation findings related to ADRC implementation approaches in three expansion sites, presents an initial evaluation of the role and functioning of the ADRC Coalition, and describes the formation of the collaboration between the AAA and the Regional Offices of MHDDAD. A framework includes benchmarks for successful collaboration development, and program logic modeling is discussed as an evaluation framework to guide future process and outcome evaluations.
