2015 State of the States in Aging and Disabilities

The results of the annual survey represent a snapshot of the current status of state government employees, the agencies that administer the programs, and the services that are provided. The 2015 survey revealed that states are continuing to address the same underlying challenges discussed in the previous reports.The report also includes summary tables that outline specific information about each individual state agency and LTSS program. Project 2020: Building on the Promise of Home and Community-Based Services

The goal of this coordinated national campaign from n4a & NASUA is to provide the resources to implement consumer-centered and cost effective long-term care strategies authorized in the 2006 reauthorization of the Older Americans Act. Empower the Aging Services Network to implement these strategies through a three-pronged program encompassing person centered access to information, evidence-based disease management and health promotion activities, and enhanced home and community-based services.

A Challenge for States: Assuring Timely Access to Optimal Long-Term Services and Supports in the Community

The Medicaid program is a major payer for long-term services and supports (LTSS) in the United States, accounting for 40 percent of total spending for long-term services and supports. This brief highlights two important aspects of operations in states that affect access to LTSS in the community: efforts to provide accurate and timely information to consumers, and procedures to make Medicaid eligibility determinations quickly and efficiently.

Aging and Disability Resource Centers (ADRC): Federal and State Efforts to Guide Consumers Through the Long-Term Services and Supports Maze

The ADRC program’s purpose is to help people of all ages, disabilities, and income levels more easily access LTSS through single points of entry, make more efficient use of care options, and maximize the services available. This publication provides background on the evolution of ADRCs, their functions and implementation, grants to states, and state and federal evaluation efforts. It also points to selected issues in continuing ADRC implementation.

Aging Strategic Alignment Project

Looking for profiles of each State’s Home-and Community-Based Service (HCBS) programs? This project report covers the components of HCBS programs under multiple funding streams. It includes an overview of findings from cross-state comparisons with accompanying tables and charts and individual profiles of 48 states and Washington, DC. The findings are intended to provide states with information that can help guide their efforts to expand and improve HCBS programs.

Real Choice Systems Change Grant Program - FY 2001 to FY 2004 Systems Change Grants: Summary Final Report

The purpose of this report is fourfold: 1) to present a brief overview of the results of the Systems Change grants awarded from FY 2001 through FY 2004, 2) to describe the remaining challenges to community living that Grantees identified, 3) to provide Grantees’ recommendations for actions needed at the state and federal level to further improve their long-term care systems, and 4) to discuss the key lessons learned regarding factors that are needed to improve states’ long-term care systems.
