Children and Nursing Facilities: Diversion Strategies and Lasting Systems Change

This study was conducted as part of the Nebraska Nursing Facility Transition (NFT) Grant. The target population included children living in nursing facilities who meet the criteria for transition to home and community-based settings and children living at home identified as at-risk for nursing facility placement. The goals of this paper were to decrease the number of children in nursing facilities, identify risk factors, and to develop a plan to keep children in home and community settings.

Nursing Facility Transition and Diversion - Module 4: Nuts and Bolts of Transition

Michigan developed a six-part training on transition and diversion. Module 4 presents the basic components of a successful transition. Again, person-centered planning is a main component of the transition and helps set the goals for the process, as well as an accurate way to gauge its success. In addition, this module discusses the need for housing, financial management, and transportation in a successful transition.

Money Follows the Person Initiatives of the Systems Change Grantees

This report highlights the work of nine CMS Money Follows the Persons Grantees, with a focus on Texas and Wisconsin. The report describes the initiatives, and discusses policy and design factors states should consider when developing MFP programs, including developing legislation and budget mechanisms for making transfers of funds, assuring availability of services and housing, identifying potential consumers for transition, developing NFT infrastructure, and monitoring and quality assurance.

Centers for Independent Living (CILs) are consumer-driven organizations, owned and operated by individuals with disabilities. New Jersey has a network of 12 CILs that provide the core services of Information and Referral, Peer Support, Independent Living Skills Training, and Advocacy. Money Follows the Person Act - New Mexico

The New Mexico legislature has passed the \"Money Follows the Person Act\" (MFP) allowing people with disabilities of all ages to choose community living. The passage of MFP dove-tails with federal legislation signed into law by President Bush in the Omnibus Budget bill. The New Mexico legislation will allow a person in a nursing facility to choose community living, and have the money that has supported the person in the facility to follow the person to the community.

The New Hampshire Home Care Connections Project: Results of the Needs Survey at Baseline Summer 2005

This baseline survey has the reviews of professionals working with those at risk of losing their subsidized housing and entering a nursing facility due to emotional, cognitive, or behavioral issues. The survey was also used to better understand and identify risk factors, and import services, associated with nursing home placement, its avoidance, and training needs.
