Long-Term Care

This is a powerpoint presentation for state staff on the NFT project. The project provides nursing home residents with information on community supports available to enable them to live in the community.

This transmittal from the Office of Health Services serves to update nursing home administrators on HB 752. This bill requires that social workers in nursing homes present residents with information on home and community-based services waivers and other programs that might help them live in the community.

In June ‘03, Maine Gov. Baldacci signed into law the Dirigo Health Reform Act. A major premise behind the law is that successful health care reform must address cost, quality, & access simultaneously & with equal vigor. Named for Maine's motto: To Lead-the program is designed to guarantee health coverage by ‘09 to those who cannot afford insurance. The ME Governor’s Office of Health Policy & Finance (GOHPF) has partnered with NASHP to develop issue briefs keeping states abreast of the reform.

The tools in this section, developed by the Michigan Department of Community Health, are intended to help ensure effectiveness and thoroughness. These tools focus on person centered planning and transition planning. Included are timelines and worksheets on housing, transportation, employement, and many other topics.

The Ohio Department of Job and Family Services (ODJFS) released this Request for Proposals (RFP) for the purpose of identifying a vendor to develop and implement the Ohio Access Success Project. This is a nursing facility transition pilot project in which targeted individuals who are aged or cognitively, sensory or physically disabled and who have been residing in a nursing facility have an opportunity to choose to transition from that setting to a community-based one.

The attached presentation entitled \"Long-Term Care: Workforce Issues for States in a Changing Society\" which was given by Robyn I. Stone, Dr.P.H. during the annual NASHP conference in Philadelphia from August 4-6. The presentation looks at who the long term care workers are and implications for the field.

This is The National Academy for State Health Policy (NASHP) report entitled State Assisted Living Policy 2002. This comprehensive report examines current regulations and Medicaid reimbursement policies for assisted living facilities in each of the 50 states. The report includes detailed comparisons of state policy and a summary of each state's assisted living regulations.

The Living and Working in the Community 2003 conference, sponsored by the Centers for Medicare and Medicaid Services (CMS), was held March 4-5, 2003. Participants included Medicaid grantees, consumers, advocates, Federal and State representatives, providers, and academics. Topics include: Quality assurance, nursing facility transition, mental health, single point of entry, workforce issues and housing issues.

The United States General Accounting Office study conducted on home and community based long-term care that is supported by Medicaid waivers. The population that they focused upon was older adults. The reasons for the study were to look at trends in uses of Medicaid waiver programs, to review quality assurance approaches used by states in their administration of the waivers, and to determine the adequacy of federal oversight of the waivers.
