Granite State Employment Project - New Hampshire

Since 2001 the State of New Hampshire has utilized MIG funds to improve its infrastructure for providing employment supports to its citizens with disabilities. Through this grant, New Hampshire has built a comprehensive strategic plan to address systemic barriers to creating a lasting transformation of its service system. The Granite State Employment Project(GSEP) and its subsequent evaluations, could serve as a valuable resource for other states. Both are available for download here.

IndependentChoices Procedure Manual - Arkansas

An alternative to the traditional care delivery system, Arkansas’ IndependentChoices was the first program nationally that offered participant directed care as an option to its’ Medicaid population. Analysis of Medical Expenditures and Service Use of Medicaid Buy-In Participants, 2002-2005

Review the key findings of this report which uses information from multiple state and federal data sources and provides the most comprehensive information to date on patterns of Medicaid and Medicare spending and service use among Buy-In participants. Overall findings suggest that Buy-In participants who are working may require fewer services or a less expensive mix of services than other adult disabled Medicaid enrollees.

Reducing Nursing Home Use Through Consumer-Directed Personal Care Services

This article focuses on the effect of Arkansas' Cash & Counseling demonstration on nursing facility use and expenditures. Medicaid Costs Under Consumer Direction for Florida Children with Developmental Disabilities

This study of Consumer Directed Care, Florida's Cash & Counseling Demonstration program for children, examines the ways in which consumer direction affects the cost of Medicaid personal care services, as well as the cost and use of other Medicaid services. Outcome measures were drawn from Medicaid claims data for 1,002 children for the first two years post-enrollment.

Leveraging Philanthropic Investments to Advance Policy Change

This article by the Assistant Secretary for Aging, Josefina G. Carbonell, describes how the US Department of Health and Human Services' Administration on Aging leveraged the investments of a number of private foundations and entered into a partnership with them to develop, test, and support the initiatives that were eventually included in the Bush administration's Choices for Independence Reauthorization Proposal.
