Facilitating Community Transitions for Dually Eligible Beneficiaries

This case study explores how health plans can help rebalance LTSS for dually eligible members. It looks at two California health plans who successfully transitioned dually eligible members from institutional to community settings. The case study uses these two examples to identify key elements for supporting dually eligible individuals with LTSS needs during the transition to living in the community.

Evidence for the Impact of the Money Follows the Person Program

This report looks at how states with strong MFP programs compare to states without or with limited MFP programs. Findings show that states with more robust MFP programs reduced utilization of institutional LTSS at a higher rate than other states. In addition, these states also rebalanced their LTSS systems more efficiently compared with other states.

Expansion Medicaid Transitions Guide

This guide is intended to educate state advocates nationally about the expansion Medicaid-to-Medicare transition process, assist them in troubleshooting problems their clients are facing, and guide them as they explore potential improvements to current processes. State advocates can use this guide as a roadmap to help residents access Medicare and other insurance programs and ensure that Medicare beneficiaries receive the benefits for which they are eligible.

Bridging the Health and Housing Gap Transitioning Medicaid Recipients from Institutions to the Community in the Context of Housing Shortages and Affordability

This report reviews federal health and housing policies, noting the barriers created that seniors with clinically-driven housing insecurity must navigate. It also reviews a series of initiatives that ACAP members are undertaking to address housing as a social determinant for this vulnerable population, with examples from California, Ohio, and North Carolina.

Toward Seamless Coverage: Identifying Enrollment Gaps and Opportunities in Medicare Transitions for People with Expansion Medicaid

This new report developed by the Medicare Rights Center for the National Council on Aging shares how some states are working to ensure seamless coverage for low-income individuals transitioning from expansion Medicaid to Medicare. The report highlights promising state-specific practices that other states might adopt, as well as barriers that states will work through in their own ways, seeking the most seamless coverage possible for lower-income older adults and people with disabilities.

Financing Transition Services When Everyone is the "Payer of Last Resort"

The Employment Policy and Measurement Rehabilitation Research and Training Center, with funding from the National Institute on Disability and Rehabilitation Research, published a report on financing transition services. Multiple funding streams, or braiding of resources, are used to fund the services and supports available for transition-age youth seeking employment or higher education. This report examines who the payers of last resort are based on different regulatory and legal requirements.
