Community-Based Organizations

A directory of evidence-based dementia support programs for informal caregivers is now available online, released in partnership between Benjamin Rose Institute on Aging and Family Caregiving Alliance (FCA). People who provide care for friends or family members with dementia can use the directory to identify programs that: Are available in person, online, or over the phone; Reduce caregiver stress and improve well-being; Increase confidence in caregiving Community-based organizations that serve informal caregivers may also find the directory helpful for conducting side-by-side comparisons of programs and or determining which evidence-based support program to adopt.

This week, the Office of Disability Employment Policy (ODEP)-funded Employer Assistance and Resource Network on Disability Inclusion (EARN) published an article titled, “Community Partnerships: An Effective Inclusion Strategy,” which includes resources, guidance, and successful case studies for inclusion-minded businesses looking to hire, support, and retain employees with disabilities. This resource provides information on diverse avenues for businesses to connect with and support employees with disabilities, including State Vocational Rehabilitation Agencies, community-based organizations, state and local workforce development systems, and educational institutions and organizations...

Scripps Gerontology Center and the Aging and Disability Business Institute have released a report detailing findings from the 2021 CBO-Health Care Contracting Request for Information (RFI) Survey. The report identifies key findings, contracting patterns and trends from the analysis of all four RFI surveys. The data provides insight on the proportion of CBOs contracting with health care entities and the status of the CBO-health care contracting market.

CHCS senior program officer, Diana Crumley, JD, MPAff, has helped state Medicaid agencies, health plans, and community-based organizations think through issues regarding health-related social needs and social care integration over the last four years. This blog post outlines four lessons from her experience supporting health care stakeholders address health-related social needs and health inequities, while focusing on advanced primary care and Medicaid managed care.

This fact sheet outlines strategies to help Medicaid stakeholders encourage partnerships between community-based organizations and health care organizations. States can provide: (1) financial support to build and sustain program capacity; (2) assistance in identifying metrics for evaluation; (3) incentives to providers to address social determinants of health; and (4) use of policy levers, including value-based contracts, managed care organization regulations, and state plan amendments.

The Partnership for Healthy Outcomes, a project of the Center for Health Care Strategies, Nonprofit Finance Fund, and the Alliance for Strong Families and Communities, has published a report that captures and analyzes lessons from health care organizations and community-based organizations (CBOs) that are partnering to address both the clinical and the social determinants of health. This report is based on a survey of more than 200 health care related partnerships.
