Community-Based Organizations (CBOs) Success Stories

The Aging and Disability Business Institute at USAging are highlighting Success Stories between community-based organizations (CBOs) and health care partners. These Success Stories provide an opportunity to learn about best practices in contracting with health care and how partnerships are improving the well-being of older adults and people with disabilities. Most recently highlighted is the story of the Southern Alabama Regional Council on Aging (SARCOA), who is the recipient of The John A. Hartford 2024 Business Innovation Award for its program that integrates health coaches into chronic care and transition management, aligning health and social care.
