Supporting Older Adults in Rural Communities: Home- and Community-Based Care

Rural communities are home to more than 20% of older adults across the country and face significant barriers to health care access, including limited transportation, care facility closures, and ongoing workforce shortages. Despite these challenges, most older adults prefer to age in place, and achieving this goal will require targeted strategies that build on the strengths of rural aging to support aging at home.

The Center for Health Care Strategies (CHCS) has recently released a new “Evidence-to-Action Collection” featuring three care models that can support personalized, community-based care for rural older adults. The three models include the Program of All-Inclusive Care for the Elderly (PACE), community-based palliative care, and community paramedicine, all of which can be tailored to the strengths and culture of rural communities.
