Accountable Health Communities (AHC) Model Evaluation

The Centers for Medicare & Medicaid Services has published the final external evaluation report for the Accountable Health Communities (AHC) Model. The AHC Model operated between 2017 and 2023 and navigated Medicaid and Medicare beneficiaries who screen positive for at least one of five social needs to community resources.
The final evaluation report found that the AHC Model:
• Decreased total costs by 3% for Medicaid beneficiaries and 4% for Medicare beneficiaries, generating more than $200 million in net savings
• Reduced hospitalizations for Medicaid members by 3%
• Reduced ambulatory-care-sensitive hospitalizations for Medicare members by 8%
• Reduced avoidable emergency department visits by 3% among Medicaid beneficiaries and by 5% among Medicare beneficiaries
The evaluation report also found that the impacts of the AHC Model were stronger for Medicaid beneficiaries with multiple needs and for Medicare beneficiaries with transportation needs.
