The Aging and Disability Resource Center Grant Program, was developed to assist states in their efforts to create a single, coordinated system of information and access for all persons seeking long term support. This toolbox has been developed for ADRC grantees to: 1) List and succinctly explain the AoA-CMS requirements per each topic, and 2) provide materials that will support the design, development and operation of resource centers.

The development of this Best Practices Manual is a pilot project to demonstrate a step-by-step process of re-integrating long term nursing home residents from residential living back into the community. This innovative Project is the result of an initiative made possible under the Passages Program sponsored by the Arkansas Division of Aging and Adult Services, in 2000-2003.

This guide is a workbook for consumers that are transitioning or wish to transition out of a nursing home. The guide provides information on programs, housing, home modifications, etc. and provides space for the consumer to fill in the steps they need to take in each category. Created for consumers in Alabama, this guide will be useful for consumers in other states as well, although contact information and specifics will not apply.

The tools in this section, developed by the Michigan Department of Community Health, are intended to help ensure effectiveness and thoroughness. These tools focus on person centered planning and transition planning. Included are timelines and worksheets on housing, transportation, employement, and many other topics.

The tool is designed to help consumers and policy makers determine the extent to which state home and community based service programs offer consumers opportunities to make choices and direct their services.

This working document provides strategies for making meetings and conferences truly accessible. It includes guidelines for ensuring that appropriate accommodations are identified and provided with respect to meeting spaces and materials, communication, room and board, and transportation. The document also provides suggestions for responding to specific functional issues, as well as sample documents and a list of additional resources.
