MDS 3.0 has been designed to improve the reliability, accuracy and usefulness of the MDS, to include the resident in the assessment process and to use standard protocols used in other settings. This website offers a number of resources regarding this tool, including videos, fact sheets, presentations and CMS training materials.

Find a clearly written, user-friendly manual developed for use by an individual transitioning into greater independence and adulthood. The toolkit provides comprehensive guidance on planning for personal assistance services and transitioning to independent living. A number of personal stories are included, along with useful questionnaires, checklists, goal development guides, sample plans, activities of daily living worksheets, and extensive PAS hiring, management and training information.

Check out this guide geared toward the participant/consumer on how to hire and manage personal assistants. Written with the goal of promoting self-determination, this user-friendly guide helps the consumer understand their role as employer and covers topics such as determining the supports they need, finding workers and the role of the Fiscal Intermediary. It includes sample forms, worksheets, checklists, job descriptions, as well as a glossary and tips for helping to keep organized.

Are you looking to develop resources for a Veteran-directed HCBS program? Find materials developed for Southern Maine’s program, including a marketing brochure, a detailed guidebook defining concepts such as self-direction and financial services, forms and instructions for emergency backup, safety and prevention strategies, care plan development and budget worksheets, and detailed information and forms to help veterans in their role as an employer.

Listen to a recording of a webinar presenting strategies for program administrators to consider as they determine how to provide FMS in their Community Living and Veteran Directed programs. Topics include how to decide on a model of FMS, whether to contract for Financial Management Services, provide them "in-house" at the AAA, or use a hybrid approach. Accompanying tip sheets and documents are included.

Looking to develop guidance materials for veterans participating in a VD-HCBS program? Here are samples from Washington’s program, including a handbook with information on meeting program staff, developing a spending plan and working with FMS. Also find a Policy & Procedure Handbook including an overview of the program, desired outcomes, eligibility, referral procedures, forms and protocols for assessment, enrollment and reimbursement.

Are states providing people with disabilities the necessary tools to reach their career goals? As part of their Disability Employment Education Project, Nevada created an Employment Education Fair, designed to develop skills and knowledge on how to be competitive in the job market. Samples of marketing material, an evaluation form and a press release about the EmployAbility 2009 Education Fair are attached, and could be used as templates by other MIG states.

As part of their Disability Employment Education Project, Nevada created a career exploration fair for youth with disabilities age 14-22. It gives students the opportunity to visit with representatives from a wide range of industries, learning first-hand about the necessary skills and educational requirements related to careers of interest. An invitation for schools, as well as an evaluation form for attendees is attached, which could be useful to other states.

Looking for job skills training materials? The state of Nevada created the attached work program samples. They include a class outline booklet, sample completion certificate, high school letter, job skills brochure, job skills flyer, programmatic report, resume template and short biography template. These templates could be useful to other MIG states interested in this approach to job skills training.

How are states implementing proposed transition pilot projects? Ohio created application materials for school districts interested in becoming one of two Regional Transition Pilot Projects. These projects would participate in developing and implementing an inter-agency transition planning and support project for transition-age youth with disabilities. A sample of the application materials are attached and could be a helpful guide to other states instituting pilot projects.
