Find an abundance of information on Health Coverage for Workers with Disabilities (HCWD), a Medicaid expansion in North Carolina created under the authority of the Ticket to Work and Work Incentives Improvement Act. Included are: reports on cost and participation projections; a powerpoint presentation on information gathered from workgroups; brochures on the program; and a fact sheet about the shift from personal care services (PCS) to HCWD.

The State Employment Leadership Network (SELN) is sponsored by Hawaii’s MIG program to improve employment outcomes for people with developmental disabilities. The SELN developed a work plan which outlines the objectives, barriers, processes, persons responsible, and target dates for completing goals within the disability employment arena.

This report analyzes the costs and benefits of the State of Michigan increasing the wages of Home Help workers. Home Help is one of several programs that provides for low-income individuals who need long-term care but do not want to move out of their homes. The report analyzes the effect of higher wages by focusing on two types of benefits and costs: public and private. The authors hypothesize that raising wages will reduce turnover and improve care, attracting more clients to the program.

The State of Vermont was seeking proposals from individuals or organizations interested in becoming the Medication Assistance Consultant. The deadline for submission of proposals was June 23, 2006. This RFP serves as a great example when seeking out contract work.

This report evaluates a range of respite care needs, presents various respite care services,provides proposed enabling legislation and a nonprofit organizational structure, and supplemental information potentially useful for beginning and operating a respite care program.

The overall goal of this project presented in this proposal is to determine the feasibility of adapting an existing cultural competence organizational assessment protocol for application in nursing facilities, in particular, skilled nursing settings, so that they can assess their organizational cultural competence as well as use results to develop strategies for improvement. In October 2002 a Real Choice Systems Change grant was awarded to the Connecticut Department of Social Services, and the A.J. Pappanikou Center was selected to administer the grant. The grant is designed to improve the capacity of Connecticut's communities to meet the needs of individuals with disabilities and their families by supporting the development of model communities that fully demonstrate accessible, appropriate, and quality community living opportunities for persons with disabilities.

Groton is a community committed to inclusion as evident in the opportunities that exist for residents with disabilities. The goal of this proposal is to make community activities more accessible and to change attitudes of the public.

This document is an application from the city of New Haven for a Real Choice Systems Change Grant. The application seeks to improve community opportunities for persons with disabilities through increased access, inclusion, and supportive services.
