The West Virginia Inclusion Campaign Materials

The West Virginia Inclusion Campaign was created as an initiative to counteract the effects of social bias and discrimination against people with disabilities or people living with long-term illnesses. Quality Assurance & Improvement Advisory Council Procedure Handbooks - West Virginia

The West Virginia QAI Project has produced two procedure handbooks for their program. The first is for the Aged/Disabled Waiver (ADW) Quality Assurance & Improvement (QAI) Advisory Council and the second is for the Medicaid MR/DD Waiver Quality Assurance & Improvement Advisory Council. The handbook describes the group purpose, role, membership, terms, responsibilities plus other responsibilities and expectations.

Personal Care Assistance (PCA) Program: Consumer Guide Book

This guidebook is written to provide information about Minnesota’s Personal Care Assistance (PCA) program and the many options consumers have. The state strives for a consumer directed service model. Sections in this guidebook define services, help consumers develop skills and provide resources to help take more control of the important services that allow independent living. This guidebook is current as of September 2004.

Passport to Independence - Delaware’s Nursing Home Transition Program

Passport to Independence is a federally-funded pilot project led by Delaware Health and Social Services’ Division of Services for Aging and Adults with Physical Disabilities (DSAAPD). The overall goal of the program is to identify, inform and assist nursing home residents, especially those who are Medicaid-eligible, who want to move to a new home by offering individualized case management to accomplish the goal. Housing Search Guide: Relocation from Nursing Facilities to the Community 2003

The “Housing Search Guide” was developed to assist professionals who have a role in “relocation” of people with disabilities from nursing facilities to community living arrangements with appropriate services and supports. The Guide was developed by staff of the Texas Independent Living Partnership, a program of ARCIL, Inc. funded by the federal Centers for Medicare & Medicaid Services under the “Real Choice Systems Change” program.

The boards in this kit were designed for a specific communication interaction, an intake interview to consider transition of an individual out of a nursing home. Handbook for Transition Navigators

This handbook, drafted by Jane Schiele and edited and revised by Laura Hershey (HCPF Systems Change Section), is designed to help other advocates or agencies to inform many more individuals about their right to live in the community, and to assist those who desire a transition back to their community. The report defines transition and offers the nuts and bolts of the program and lastly overs tips for moving day and beyond.

Involving People with Disabilities as Members of Advisory Groups

Service providers, schools, parks departments, public health agencies and other organizations make decisions and policies that profoundly affect the health, employment, income and well-being of people with disabilities. This capacity carries the obligation to involve the people most affected by including individuals with disabilities in the decision-making process.
