The National Center on Caregiving at Family Caregiver Alliance (FCA) has developed a new online resource. The toolkit is a practical resource targeted to practitioners and program administrators interested in integrating caregiver assessment into their work with caregiving families, older people and adults with disabilities. The new consumer-directed Medicaid program in Montana, Big Sky Bonanza (BSB) program has created the following products provided to new enrollees. The Waiver Orientation manual contains a program overview, roles and responsibilities and training process. My Support Services Plan is an 18 page worksheet to help consumers develop goal statements, capture personal, health, and spending plan, create an emergency back-up plan and list medical information.

This brochure was created using feedback from consumer focus groups, peer trainers, and other stakeholders. The brochure provides safety & prevention strategies, planning for backup care, preparing for attendant support during a community-wide disaster, how to minimize risk of theft, and health care emergency instructions and form. The strategies address emergency backup and incident management for consumers directing their own care or who have an authorized representative directing their care.

The Arkansas IndependentChoices program developed two new tools. The Consumer-Directed training manual will provide users with ideas and suggestions on hiring, training, and managing a personal care attendant. The Personal Care Attendant handbook will provide users with training material that will help trainees do their job as a personal care attendant. Both manuals contain information about hiring, communication and management issues.

The Shawnee County CDDO is pleased to present the Kansas Personal Assistance Supports and Services (K-PASS) Self-Direction Toolkit. The purpose is to provide people with disabilities the information and tools they need to hire and manage their personal assistant supports and services. The toolkit includes such items as job descriptions, training information, emergency back-up plans and other useful resources.

The manual is a nursing home to community discharge planning manual designed primarily for use in nursing homes and may be of help to others to use in developing manuals for their communities. It is designed to orient the long-term care providers to the nursing home discharge process by defining the roles of the key partners who are involved in returning residents to community living and to document a standard protocol for accessing and maximizing partner resources.

How many hoops does a consumer have to jump through to receive services? The goal of service delivery for ADRC is seamless entry into long term supports for consumers. This document outlines basic indicators of streamlining access by looking at different aspects of the process. The Measures Matrix will utilze the Evaluation Plan Highlights document also attached.

This publication, a series of 5 guides, provides technical assistance to HOME participating jurisdictions (PJ) and their housing partners on the fair housing laws and regulations that apply to HOME Program participants. The series reviews the applicable Federal fair housing requirements, illustrates best practices in promoting fair housing, and highlights PJ responsibilities for ensuring compliance with fair housing requirements.

This manual is intended to address two identified training needs for consultants/support brokers working with participants in self-directed care. MODULE ONE: Facilitating the Paradigm Shift for Consultants and MODULE TWO: The Dynamics of Choice and Decision-Making for Participants. The program identifies training needs by addressing the philosophical framework necessary for successful implementation of a participant-directed service and was designed to be delivered in two half-day sessions.

The Department of Mental Retardation (DMR) has established a system of reporting and monitoring incidents that occur with individuals served by the department in order to identify, manage and reduce overall risk for the individual. This procedure delineates a standardized process for reporting, documentation and follow-up of reportable incidents as defined, involving individuals served by the department. This reporting system helps implement individual plans and assists in quality oversight.
