RE-AIM, a framework for program developers, planners, and evaluators to use to translate research into practices and policies that maintain the health and quality of life of caregivers. This monograph argues that this issue should be a public health policy priority, summarizes RE-AIM, provides policy and program scenarios for using the framework, and list resources and tools. This web-based resource helps Medicaid stakeholders allocate resources and demonstrate the business case for improving quality in health care. It is designed for programs that aim to more effectively manage the care of adults with disabilities and chronic conditions. Because the care of this population consumes nearly 70% of Medicaid expenditures, it represents a good opportunity to improve health outcomes, reduce unnecessary utilization, and control costs. This report provides recommendations for persons with Acquired Brain Injury (ABI) or Traumatic Brain Injury (TBI) to acquire and maintain employment in Oregon. Currently, the common program for people with TBI/ABI nationally is Vocational Rehabilitation. However, this report suggests expanding services by using a supported employment model with an interdisciplinary team. Major characteristics of this model include paid employment, integrated worksites and ongoing support.
