WorkSource Wisconsin Employer Disability Resource Center

Wisconsin developed the WorkSource website and phone and email hotline to answer employers’ questions about recruiting and employing people with disabilities. Here you can find a focus group and needs assessment report used in developing the website. There are also a WorkSource brochure, a list of questions that can be answered within 24 hours, and a link to the site.

Family Education and Support in Delaware - Web-based Curriculum

The University of Delaware’s Center for Disabilities Studies created a web-based curriculum to provide helpful information on social, emotional and behavioral health problems in youth for parents/caregivers and also for the children/youth. The program can be used in individual family sessions, multifamily groups or in a school setting. The website offers a host of practical guides and handouts for families, therapist topics and session guides.

Informational Materials on Healthcare for Workers with Disabilities – Washington

Under the Healthcare for Workers with Disabilities (HWD) program, people with disabilities can earn more money but still receive Medicaid benefits by paying premiums based on a sliding income scale. At this website, the state offers eligibility information, instructions on applying for HWD coverage, and links to further information. HIV/AIDS and Older Americans Fact Sheets

HIV/AIDS is not just a young person's disease. Fifteen percent of new HIV/AIDS cases are among Americans over age 50. Many older people with HIV do not know they are infected because they have never been tested. They are not accessing the treatment they need and may be unknowingly spreading HIV to others. This fact sheet discusses HIV/AIDS among older Americans and why all sexually active Americans, including those over age 50, should be tested for HIV as part of their routine health care.

Freedom to Work Information and Marketing Materials – South Dakota

South Dakota’s Freedom to Work (FTW) project provides supports to job seekers with disabilities. Here are examples of tools used to publicize the project. The FTW brochure summarizes all associated services, while the Personal Assistance Services brochure and poster and the Work Incentives Planning and Assistance brochure provide more detailed information for those particular services. The website has relevant links and quarterly newsletters about FTW.

SCAccess Website – South Carolina

SCAccess provides this one-stop website for finding aging and disability resources, applying for benefits, and learning about rights. One section contains a directory for finding and tips for hiring the right personal care worker. Another helps individuals connect with information specialists at each Area Agency on Aging. Consumers can also download commonly used documents and applications. Finally, the site is integrated with agency case management tools.

Money Follows the Person (MFP) Website - Arkansas

The Arkansas MFP application will transition 305 individuals who have resided in institutions six months or longer into qualified home and community-based programs. This website offers program highlights, operational protocol, benchmarks participant recruitment and enrollment information and benefits and services. Users can also find a variety of stakeholder involvement opportunities including conference dates, the Governor's Task Force List and an Organizational Structure Chart.
