This on-demand training addresses nursing home diversion by reaching out to individuals new to their disabilities while they’re still in rehab facilities in the Phoenix area. Presenters shared how they built and funded the program, how it worked, and real-world examples and success stories from the program.

Community Integration: A Holistic Approach to the New Core Services for Transition & Diversion

This training addressed the new core services for centers for independent living as a natural extension of the Independent Living philosophy. Assisting people to move out of institutions – or staying out of them in the first place – is some of the most important and impactful work that CILs do. The CIL-NET organized this interactive workshop in Atlanta to help figure it all out together – as a community. The presenters provided a great deal of information and real-world guidance.

Bridging the Health and Housing Gap Transitioning Medicaid Recipients from Institutions to the Community in the Context of Housing Shortages and Affordability

This report reviews federal health and housing policies, noting the barriers created that seniors with clinically-driven housing insecurity must navigate. It also reviews a series of initiatives that ACAP members are undertaking to address housing as a social determinant for this vulnerable population, with examples from California, Ohio, and North Carolina.

Integrating Older Adult Behavioral Health Into Long-Term Care Rebalancing

The National Coalition on Mental Health & Aging (NCMHA) has released a report on integrating older adult behavioral health into long-term care rebalancing. The report includes opportunities and recommendations for for funders such as public health agencies and managed care organizations (MCOs). The report notes that close to 50 percent of persons receiving long-term care have a mental disorder other than dementia.

Medicaid HCBS Settings Regulations and Adult Services

This report details results of a survey administered to Adult Service providers to learn more about the locations, funding sources, and supports that these programs provide to seniors and people with disabilities. The report demonstrates how Adult Services include a wide array of services, including a combination of social and medical supports. ADvancing States is concerned that new CMS regulations may reduce the number of available providers in the Medicaid program.
