Fiscal Impact Statement: Development of a Disability and Aging Resource Center on Long-Term Care.

The District of Columbia is seeking to expand the availability of home and community-based services (HCBS) for the aged and adults with physical disabilities.The Disability and Aging Resource Center will represent a cost to the District in the immediate short-term, however the Center is expected to have cumulative savings by FY 2005. This file is a complete fiscal impact statement for this project.

Promising Practices in HCBS: New Jersey- Single Access Point for Information on All Services for Older People

This report explains the State of New Jersey’s single entry system for long-term supports and other services for older people, including a toll-free telephone number for information and services. The effectiveness of the system in each of New Jersey's 21 counties is evaluated through compliance with state protocols and consumer satisfaction reports. The report provides the intervention, implementation, and impact of the program.

Aging & Disability Resource Center Technical Assistance Website and Presentation

The innovative site is an important component of the ADRC Technical Assistance Exchange, housed at The Lewin Group. ADRCs are designed to overcome barriers to community living for people with disabilities of all ages by streamlining access to long-term support services through \"one-stop\" resource centers. Twenty-three states and one territory have received grants to develop ADRCs. Also included here is a presentation of the major TA activities support for the ADRC grantees.

Texoma Real Choice Program: System Navigators-Overview, Presentation, Forms and Survey

The Texoma Real Choice Program--a demonstration project made possible by a grant awarded to the Texas Health and Human Service Commission by the federal Centers for Medicare and Medicaid Services. The programs goal is to connect persons with disabilities to real service options with the help of system navigators available to assist people through systems and support groups. Increasing the Efficiency of Safety Net Services in New Hampshire by Establishing Community Care Centers in Each Municipality
