Vermont Aging & Disability Resource Connection Newsletter

The Vermont ADRC publishes this bi-monthly newsletter to keep partners and staff connected to the activities of the grant within the two local pilot regions and nationally. They will use this newsletter to highlight key activities of the grant: progress in establishing partnerships across service networks, cross training, utilization of refer and other resource databases, the formative evaluation, and community events.

Memorandum of Understanding: Medicaid Eligibility Outreach

This memorandum of understanding was signed as part of the Vermont ADRC project between the Vermont Department of Aging and Independent Living and the Department of Children and Families (Medicaid). The MOU is informative for streamlining access efforts. Other ADRCs designing programs will find this MOU a useful example.

Making ‘Cents’ of Health Care Financing: Training Curriculum on MassHealth

This power point presentation and associated brochure helps Massachusetts families with children and youth with special health needs understand MassHealth, the MA Medicaid program. This program is an umbrella for many different types of public health care. Learn about eligibility, standards, criteria, and premiums. This work is part of the 2004 RCSC CMS Grantees for Health Care Information and Education Centers.

Massachusetts Family-to-Family Health Care Program Brochures & Website

What programs do the MA F2F Information and Education Center offer? Review their brochure about the services and supports. This parent-run center provides free technical assistance about health care, public benefits and other health care financing options. The pamphlet is available in English, Portuguese and Spanish. Additional information can be found on their website. The MA Family-to-Family Health Care Information and Education Center is a 2004 RSCS CMS Grantee.

A Guide to Applying for New Mexico Waiver Programs Application Handbooks

For many families and individuals with disabilities, the process of applying for Waivers can seem complex and overwhelming. Developed by Parents Reaching Out this series brings together accurate information and advice from families, advocates, case managers and agencies, to provide a “road map” of sorts. Titles include a Comparison Guide for HCBS Waivers, a DD Waiver Application Handbook, Guidebook to Supplemental Security Income and the Book of Acronyms. Titles are also available in Spanish.

Family Health Care Tips & Fact Sheets – New Mexico

The New Mexico Family to Family grantees, Parents Reaching Out, have put together over a dozen tips sheets. Topics include Advocating for Yourself, Teen Scene: Communication with Health Care Providers, Preparing for Doctors Visits, Waivers, Navigating Medicaid, SSI, Preventing Medical Errors, Transitioning Health Care, HIPAA, and FAQ sabot Mi Via and EPSDT. Also includes is a brochure called Telling Your Story, Tips for Sharing your Experiences. All the fact sheets are also available in Spanish.

Georgia Aging and Disability Resource Connection Expansion Evaluation

This report describes the process evaluation findings related to ADRC implementation approaches in three expansion sites, presents an initial evaluation of the role and functioning of the ADRC Coalition, and describes the formation of the collaboration between the AAA and the Regional Offices of MHDDAD. A framework includes benchmarks for successful collaboration development, and program logic modeling is discussed as an evaluation framework to guide future process and outcome evaluations.

University of Connecticut - Center for Excellence in Disabilities Website

Established in 1985, the goal of this Center is to develop, implement, and disseminate a coordinated group of applied research, demonstration and training projects directed at meeting the needs of persons with disabilities and their families in community based settings.
