Quality

This is the keynote presentation given by Michael Kendrick at The Promise of Opportunity conference in Albany, NY. The presentation includes a discussion of person-centered planning vs. person-centeredness. It also looks at the relationships and values that must be present if people are to be at the heart of social service systems.

This guide is designed to provide states and programs involved in consumer-directed services with a practical handbook on ensuring and improving the quality of services. This guide is based on a philosophy that the views of the major program stakeholders – consumers, families, program staff, regulators, funders – are the necessary starting point for the design of a quality system.

Medicaid beneficiaries who have disabilities and receive personal care services (PCS) from home care agencies have little control over their care. As a result, some are dissatisfied, have unmet needs, and experience diminished quality of life. This study of Arkansas’s Cash and Counseling demonstration program, IndependentChoices, examines how consumer direction affects these aspects of care quality relative to agency-directed services

This report describes the State of Washington’s program to increase the support of family caregivers for people with Alzheimer’s disease in ethnically diverse communities. The program includes outreach, education, and culturally sensitive assessment. The implementation and impact are described in this report.

On May 28, 2004, Glenn Stanton Acting Director of CMS’ Disabled Elderly Health Programs Group transmitted modifications and additions to the Interim Procedural Guidance for oversight of Medicaid waivers to all CMS Regional Administrators. The Guidance documents include the memo from Mr. Stanton, a diagram detailing the HCBS Waiver Quality Life Cycle and several attachments. The Guidance was updated in Feb. 2007, the link to that letter is also attached.

The Formative Evaluation of the Real Choice Systems Change Grants evaluates how the Systems Change grantees are progressing towards meeting their goals. The Formative Evaluation identifies problems faced by the grantees and ways that they deal with the problems. This document provides summary information about each of the FY2003 Grantees.

In fiscal year (FY) 2004, Congress appropriated approximately $40 million in funding for a new round of Real Choice Systems Change Grants. With this solicitation, the Centers for Medicare & Medicaid Services (CMS) invites proposals for grants totaling approximately $31 million of these funds.

This document looks at Colorado's home and community based services. Specifically, the report focuses on single point of entry and single access points.
