What is the vision of “quality” in community support for people with developmental disabilities? Quality is a concept of many dimensions. This website is your invitation to join the quality community in its journey to fulfill the vision of a positive future for all people with developmental disabilities. The site offers webinars, resources, publications, tools and links.

The Virginia Real Choice Systems Change Grantees have developed a website designed to help individuals with disabilities and those who support them learn more about self-direction. The site is a place for self-advocates to find training modules, waivers workbooks, person-centered resources, and other materials related to consumer-directed services.

North Carolina's Division of Mental Health, Developmental Disabilities and Substance Abuse Services has posted a number of great resources to their website related to quality. A sampling of the forms, manuals and recent reports includes: The Quality Management Survey, a Consumer Satisfaction Survey, forms on Critical Incident, Quarterly Reports, and reporting guidelines for local management entities in their annual Quality Improvement Reports.

This is the first newsletter of the National Core Indicators (NCI) program. The publication is designed to keep you abreast of developments and best practices in performance and outcome measurement as exemplified in initiatives among NCI participants. Issue 1, Volume 1 explores Self-Advocates in Vermont, Surveys used in California and Pennsylvania, and a new video from Kentucky called \"Team Up for Success\".

The Quality Choices for Maine project was conducted from 2001-2004. This report focuses on Access to Services, one part of the three-year RCSC Quality Grant. The specific housing related goal was “to improve access to community housing for people with disabilities of all types.” Project activities were overseen by the Access to Services Technical Assistance Group and a variety of stakeholders. As part of the State of Wisconsin’s Quality Close to Home (QCTH) project, a contract was established with the University of Wisconsin – Madison’s Center for Health Systems Research and Analysis (CHSRA). This report describes the uses, methods, advantages, and disadvantages of four distinct sets of quality indicators, also called personal outcomes and performance outcomes. It also provides a overview, structural review and observations of the quality indicators.

The Ohio Department of Mental Retardation and Developmental Disabilities (ODMR/DD), a 2003 Real Choice Systems Change QA/QI in HCBS grantee, has begun to develop with initial stakeholder input, a Quality Framework to guide what data is collected. Ohio's QA/QI in HCBS grant application was written specifically to fund the design, development and implementation of all aspects of a quality information management system. This presentation reviews important changes CMS is making in the reporting system for FY04 RCSC grants. The slides provide an overview of the “why and how of the proposed changes” offers next steps and introduces the Logic Model Framework. The Logic Model is a tool for strategic planning and program evaluation. This presentation notifies grantees of upcoming changes to the web-based reporting system and introduces the systems terminology, context and concepts.

This report describes the FY 2002 and FY 2003 Grantees’ challenges and progress using information provided by the Grantees during the reporting period October 1, 2003 to September 30, 2004. The Wisconsin Department of Health & Family Services is designing an integrated state-local quality management system for the state’s HCBS long-term care programs. The Long Term Care Functional Screen (LTCFS) is a key element of their program, used for eligibility determination and rate settings. This paper explores ways to discover accuracy in the Level of Care determination process and describes an approach to LTC FS building on enhancements of current efforts and incorporating new activities.
