The West Virginia Medicaid Aged and Disabled Waiver Program developed a number of provider audit tools. Monitoring Tools include: Site, Homemaker, Case Management, Personnel Files and Quality Assurance.

West Virginia has chosen to modify the PES E/D to capture both the experiences of participants in the Personal Options waiver (who are able to self-direct a portion of their services), as well as those in the traditional waiver program. WV added an additional domain for Personal Options participants that addresses the adequacy of support staff, resource consultants and financial management services. The survey and training manual are attached.

Minnesota has shared two of their protocol surveys created as a follow-up to a more extensive, detailed gap analysis completed statewide in 2001. This update has been completed in 2003 and 2005, and will continue to be part of HCBS capacity assessment and HCBS capacity building. The analysis looked at the availability and demand of specific services, including long-term care supports, senior housing and long-term care beds. What areas of quality are important to individuals with disabilities, families and stakeholders in Ohio? The framework illustrates the areas of quality to be addressed by a functioning quality information management system. Organized by Domains, each section is accompanied by an Outcome. Next, the Core Indicators express how the system will know if the Outcome is present. Performance Measures establish the criteria for measurement and calculation. The Ohio Department of Mental Retardation and Developmental Disabilities (ODMRDD) Division has issued an RFP for a contractor to implement recommendations received through another consultant for their Marketing and Communication plan and to implement a recently completed Communication Planning Tool. The plan is specific to educating all constituents on Quality Management concepts and how information resources will assist in making decisions affecting quality of life. RFP closes 10/20/06.

This is a comprehensive report detailing the results of Maryland's 2004 and 2005 Participant Experience Survey (PES) results. The PES is a tool, developed by Medstat that measures the experiences of participants of 1915c waivers. The Maryland Department of Health and Mental Hygiene used funds from a 2001 Real Choice Systems Change Grant to survey two Maryland waivers, the Living at Home Waiver and the Older Adults Waiver.

The Ohio Department of Mental Retardation and Developmental Disabilities (ODMR/DD) is issuing this Request for Proposals (RFP) to engage a contractor in a partnership to review the current Individual and Family (Draft) Survey items, recommend changes as needed to complete the development of the survey, establish the reliability and validity of it’s measures, ensure integration with the Core Indicators identified within Ohio’s Quality Framework, and develop a final draft of the survey.

This report describes grant activities in three major LTC systems areas: Access to Long-Term Care Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure. For each of the focus areas the report describes Grantees’ accomplishments, illustrates the challenges and discusses the consumers’ role in the implementation and evaluation of activities. The report uses information provided during the reporting period October 1, 2004 to September 30, 2005.

This report highlights the work of nine CMS Money Follows the Persons Grantees, with a focus on Texas and Wisconsin. The report describes the initiatives, and discusses policy and design factors states should consider when developing MFP programs, including developing legislation and budget mechanisms for making transfers of funds, assuring availability of services and housing, identifying potential consumers for transition, developing NFT infrastructure, and monitoring and quality assurance.
