This guide is a supplement to ABCs of Nursing Home Transition: An Orientation Manual for New Transition Facilitators. The two guides are meant to be used together. This second guide, gives greater detail on one of the most challenging aspects to the transition process—locating and securing housing that meets the individual’s needs and preferences so that the transition can be completed. The Appendix offers tools for housing specialist and consumers including checklists and how-to fact sheets.

This publication provides a summary overview of the accomplishments and enduring changes brought about by all of the Systems Change NFT Grantees Despite bringing about many enduring systems improvements, Grantees reported that many transition barriers remain. Grantees made recommendations to help states address continuing barriers to nursing facility transition and diversion so that no one need live in a nursing home simply because sufficient community services and supports are unavailable.

The first in a series on housing issues, this brief provides an overview of various federal and state housing financing programs that can be used to support low income workers with disabilities. The brief also summarizes the related nature of housing and employment services and the impacts of the current housing market on employees with disabilities. The series is meant to provide information to MIG grantees and help them create linkages with agencies providing direct housing services.

This report presents findings from Oct ‘05 – Oct ‘07, which includes the first nine months of strategic planning for the FY05 and FY06 Systems Transformation (ST) Grants and the subsequent initial 15-months of implementation (through Oct ‘07) for the FY05 ST Grants. The report focuses primarily on documenting grantees' experiences during the start-up period and grantees' progress during early implementation and suggests early trends and findings.

The authors offer recommendations for providing successful career preparation support to youth with mental health needs. They studied five successful programs to identify common design features that address frequently identified challenges in transitioning to an independent adulthood. They also offer analysis of systems level factors affecting program design and sustainability, in addition to recommendations for staff and policymakers.

This publication presents lessons from financing projection models of long-term care (LTC). It describes the methods, conclusions, strengths and weaknesses of several models. This analysis examines how U.S. federal laws may be affected if the U.N. Convention on the Rights of Persons with Disabilities (CRPD) is ratified by the country. The authors summarize each CRPD article along with with U.S. laws that either guarantee the same rights or contradict the convention. The authors conclude that better implementation of existing laws or congressional action would bring federal law in line with CRPD.

State leaders are interested in the experience of states that developed adult foster care as part of their service array, trends in provider supply, regulations governing providers, and quality oversight practices. This report is based on the policies and practices in five states – Arizona, Maine, Oregon, Washington and Wisconsin. The states were selected based on their approach to licensing and Medicaid coverage for this residential option. The report focuses on programs for older adults.

This paper outlines the most common barriers to implementing livable communities and outlines cases where these barriers were overcome using planning and zoning tools. Housing, transportation and mobility, land use, cooperation and communication, public involvement in community planning, and leadership are the components of livable communities that are addressed.

This article explores the benefits and challenges people who have disabilities experience when managing their personal care programs. It also examines how LIFE Accounts would complement social participation opportunities for persons with disabilities.
