Self Determination in Long Term Care (SDLTC) is a MI Choice waiver program option. The following forms are used for this program: Budget worksheet, Disenrollment form, Right to Hire form, Right to Hire with Driving Record, Job description form for a PCA/HHA, and Backup Worker form.

Two presentations are included here: "It is so Ordered: Modernizing Michigan Medicaid Long Term Care" and "Principles and Practices of Person-Centered Planning: Detroit Area Agency on Aging".

Michigan has developed this information to clarify the model of Agency with Choice in their states. Agency with Choice is one model for supporting individuals in the MI Choice Waiver who are aged or have disabilities.

This Organizational Readiness Structure was developed for the Self Determination in Long Term Care program. It includes staff, workforce, participant, and quality components.

Contained here are materials used for the readiness review of the fiscal intermediary for Michigan's Self-Determination in Long-Term Care program. There is an Overview of the Readiness Review Protocol, the Readiness Review form, an Interview Protocol, and an Appendix for additional information.

A summary from the June 2001 symposium titled, “Independent Choices: A National Symposium on Consumer-Direction and Self-Determination for the Elderly and Persons with Disabilities”. Major themes and outcomes are highlighted based on the dialogue from the various speakers including policymakers, program administrators, consumer advocates, service producers, and researchers.

This presentation prepared by the Autism Society of Micigan outlines the cornerstones of Person Centered Planning. In addition to evaluation examples, suggestions for working with different populations in the process are provided.
