Data Resource Center on Children and Youth with Special Health Care Needs (CYSHCN) Website

This site provides data, tips and tools to use with data for states and families to help guide improvements in community-based systems of care for CYSHCN. You will find a variety of resources on this site - including an easy to use, interactive data query feature that allows users to view and compare state, regional and nationwide findings from the National Survey of Children with Special Health Care Needs.

Systems Change Grants for Community Living: Compendium Third Edition

The Compendium contains basic information about each of the Real Choice Systems Change Grantees. The Compendium will help you learn more about how these grants will be used to allow more people of all ages with a disability or long term illness to live and participate in their communities. Real Choice Systems Change Grantees will also find the Compendium useful to identify other Grantees with similar goals and activities. This edition of the Compendium replaces all previous editions.

Work Book: Improving Quality in Home and Community-Based Services and Supports

The HCBS Work Book is a technical assistance guide to assist state agencies with their HCBS quality improvement efforts. It provides step-by-step guidance on how to conduct quality improvement projects within the HCBS waiver program, and illustrates the principles of quality improvement in an easy to use format. The Work Book was developed for CMS by the Muskie School of Public Service at the University of Southern Maine.

Technical Assistance Provider Contact Information

Beginning in 2001 and continuing in 2002 and 2003, technical assistance for the Real Choice Systems Change grantees has been provided by the Exchange Collaborative team (combined efforts of ILRU and Rutgers University/CSHP). These documents provide contact information for the TA providers and two charts indicating which Exchange Team is responsible for providing TA to the grantees. In 2002 & 2002, TA was divided according to states. In 2003, TA was distributed according to theme.

Maine\'s Plan Development Work Group for Community-Based Living Homepage

The Work Group includes both consumers and representatives from five departments within state government. The five departments represented include: the Department of Human Services, the Department of Behavioral and Developmental Services, the Department of Education, the Department of Labor and the Department of Corrections. The site provides links and information to resources, reports, and listserves dedicated to Maine's Plan Development Work Group for Community-Based Living.
