Medicaid HCBS Waiver Expenditures FY 2001 through FY 2006

This memorandum and tables present data on Medicaid 1915(c) Home and Community-Based Services (HCBS) waiver expenditures from Federal Fiscal Years 2001 through 2006. Each HCBS waiver is classified by population served in order to show the distribution of HCBS waiver expenditures across long term care populations. This year's memo also identifies which waivers exclusively serve children.

Peer Mentor’s Mentoree Training & Resource Manuals

The Health Benefits for Workers with Disabilities contracting with Valek & Co. to design and provide services relating to the recruiting, training and matching of mentors/mentorees for those interested in sharing their experience about rejoining the workforce. Included are two versions of recruitment letters, phone scripts, booklet and flyers. They received 23 mentor applications and 19 mentoree applications. Review the extensive training materials, exit surveys, and final reports.

Health Benefits for Workers with Disabilities (HBWD) Website: Illinois

Illinois has joined the national movement to help people with disabilities return to work. The new program called Health Benefits for Workers with Disabilities (HBWD) started providing medical benefits in January 2002. Visit their site which provides program information and statistics, application and instructions, brochure and radio ads, FAQs, presentation requests and links, E-Pay, eligibility requirements, premium cost & co-pays, Rx Buying Club, plus information in Español.

Medicaid Long Term Care Expenditures FY 2006

This memorandum presents data on Medicaid long-term care expenditures in Federal Fiscal Year 2006 (Oct 2005-Sept 2006). Table 1 presents national data on Medicaid expenditures for Long-Term Care from 1994-2006. Real Choice System Change Grant Program: Fifth Year Report - Progress and Challenges of the FY 2004 Grantees (October 1, 2005 – September 30, 2006)

This report describes grant activities (accomplishments and progress) in three major LTC systems areas: Access to LTC Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure. Medicaid: A Primer

Money Follows the Person Rebalancing Demonstration 2007 Awards Press Release

Thirteen states and the District of Columbia will get more than $547 million in grants over five years to build Medicaid long-term care programs that will help keep people at home and out of institutions. This “Money Follows the Person” initiative was included in the Deficit Reduction Act of 2005 (DRA), currently being implemented by CMS. The press release lists the states and the amount of money committed over the next five years.

Real Choice Systems Change Grant Program - FY 2001 Community-Integrated Personal Assistance Services and Supports Grantees and Real Choice Grantees: Final Report

This report provides an overview of 35 states’ initiatives to improve their LTC systems and includes lessons learned and recommendations that can guide states undertaking similar initiatives. Despite accomplishments and improvements, Grantees described continuing barriers including insufficient funding for home and community services; workforce shortages; lack of affordable and accessible housing and transportation; and outdated or inflexible administrative, statutory, and regulatory provisions.

Choosing Independence: An Overview of the Cash & Counseling Model of Self-Directed Personal Assistance Services

In light of this major change and assuming that states may want to learn more about Cash & Counseling, this publication offers an in-depth description of the option, a discussion of key findings from the three-state demonstration, and preliminary information about the program’s expansion into 12 more states. Real Choice Systems Change Grant Program: First Year Report

To help States build the infrastructure needed to remove barriers and create more consumer responsive LTC systems, CMS awarded $70 million in Real Choice Systems Change Grants. Fifty-two Grants were awarded in Sept. 2001 (25 Real Choice, 10 Community-Integrated Personal Assistance Services & Supports, 17 Nursing Facility Transition). This report describes the progress FY01 Grantees have made on their activities during the first year of the three-year Grant period (Oct 1, 2001-Sept 30, 2002.)
